整合式心理治疗联合经颅磁刺激在广泛性焦虑障碍中的应用分析

摘　　要：目的探讨整合式心理治疗联合重复经颅磁刺激(rTMS)在广泛性焦虑障碍(GAD)患者中的应用效果.方法2018年7月至2019年12月68例GAD患者,采用随机数字表法分为对照组与观察组(各34例);对照组常规医嘱用药联合常规护理,观察组在对照组基础上采用整合式心理治疗联合rTMS治疗.观察比较两组患者治疗前后焦虑情绪状况(广泛性焦虑障碍量表(GAD-7)、汉密尔顿焦虑量表(HAMA))、生存质量(世界卫生组织生存质量测定量表(WHOQOL-100))及治疗过程中发生的不良反应.结果治疗前两组患者GAD-7、HAMA评分比较,差异均无统计学意义(P>0.05);治疗后观察组GAD-7、HAMA评分均低于对照组,差异有统计学意义(P<0.05);治疗前两组患者WHOQOL-100评分比较,差异无统计学意义(P>0.05);治疗后观察组心理、社会关系、独立性与周围环境评分均高于对照组,差异有统计学意义(P<0.05);两组患者不良反应发生率比较,差异无统计学意义(P>0.05).结论整合式心理治疗联合rTMS治疗GAD可有效改善患者焦虑状况,提升生存质量,安全性高.Objective To investigate the ffects of integrated psychotherapy combined with repetitive transcranial magnetic stimulation(rTMS)in patients with Generalized Anxiety Disorder(GAD).Methods 68 GAD patients admitted to Taizhou second people's hospital from July 2018 to December 2019 were prospectively selected as the research objects.The random number table method was used to divide the control group and study group(34 cases each).In the control group,conventional medicine was prescribed in combination with routine nursed.In the research group,integrated psychotherapy combined with rTMS was used on the basis of the control group.The Generalized Anxiety Disorder(GAD-7),Hamilton Anxiety Scale(HAMA),quality of Life(The World Health Organization Qualty of life-100(WHOQOL-100))and adverse reactions during treatment were observed and compared between the two groups.Results There was no statistically signifcant difference in GAD-7 and HAMA scores between the two groups before treatment(P>0.05).After treatment,GAD--7 and HAMA scores in the study group were lower than control group,with stristically significant differences(P<0.05).There was no significant difference in WHOQOL-100 score between the two groups before treatment(P>0.05)。After treatment,scores of psychology,social relationship,independence and surrounding environment in the study group were higher than control group,with staistically significant differences(P<0.05)。There was no stistically significant diference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Integrative psychotherapy combined with rTMS for GAD can effectively improving anxiety,improving quality of life with high safety.
