清咽利膈汤加味治疗急性化脓性扁桃体炎的疗效研究

摘　　要：目的观察清咽利膈汤加味治疗急性化脓性扁桃体炎的临床疗效。方法患者106例随机分为对照组和治疗组,对照组给予常规治疗,治疗组在对照组的基础上加用清咽利膈汤加味口服治疗,疗程为5 d。分析比较两组的临床疗效,治疗前后关节症状评分、中医证候积分、症状(发热、咽痛、扁桃体肿大、脓性分泌物)消失时间、肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、超敏C反应蛋白(hsCRP)、白细胞计数(WBC)等炎症因子水平。结果治疗后,治疗组总有效率为98.11%,显著高于对照组的88.68%(P<0.05);治疗后两组的中医证候积分均较治疗前显著降低(P<0.05),且观察组治疗后中医证候积分低于对照组(P<0.05);观察组发热、咽痛、扁桃体肿大、脓性分泌物等症状消失时间均短于对照组(P<0.05);两组治疗后血清TNF-α、IL-1β、IL-6、hs-CRP、WBC水平均较治疗前降低(P<0.05),且观察组治疗后上述血清炎症因子水平均低于对照组(P<0.05)。结论清咽利膈汤加味治疗急性化脓性扁桃体炎可以提高临床疗效,缩短疾病恢复时间,改善炎症反应。Objective:To investigate the clinical effects of Modified Qingyan Lige Decoction on acute suppura⁃tive tonsillitis.Methods:106 patients were randomly divided into the control group and the treatment group.The control group was given conventional treatment,and the treatment group was treated with Modified Qingyan Lige Decoction on that basis.The treatment course was 5 days.Clinical efficacy of the two groups,joint symptom score before and after treatment,TCM syndrome score,disappearance time of symptoms(fever,sore throat,tonsil enlarge⁃ment,purulent secretion),TNF-ɑ,white blood cells levels of inflammatory factors such as IL-1β,IL-6,hs-CRP and WBC were analyzed and compared.Results:After treatment,the total effective rate in the treatment group was significantly higher than that in the control group(P<0.05);TCM syndrome scores of the two groups after treatment were significantly lower than before treatment(P<0.05),and the treatment group reduced more(P<0.05).The scores were lower than those in the control group(P<0.05).Fever,sore throat,tonsil enlargement and purulent secretions in the treatment group disappeared in a shorter time than in the control group(P<0.05).The levels of TNF-α,IL-1β,IL-6,hs-CRP and WBC in both groups were lower than before treatment(P<0.05),and the serum inflammatory factor levels in the treatment group were lower than those in the control group after treat⁃ment(P<0.05).Conclusion:Modified Qingyan Lige Decoction on acute suppurative tonsillitis can improve the clinical efficacy,shorten the disease recovery time,and improve the inflammatory response.
