摘　　要：目的探讨腓骨蛋白1(fibulin-1,FBLN1)在胃腺癌组织中的RNA表达量差异及其对胃腺癌诊断和预后价值。方法收集2018年1月至2019年9月安徽省铜陵市人民医院30对癌/癌旁样本,以及GEO(gene expression omnibus)数据库样本(GSE66229数据集),对其FBLN1表达量进行差异分析和ROC分析。在癌症和肿瘤基因图谱(cancer genome atlas,TCGA)数据库的胃腺癌数据中对FBLN1进行生存分析和微卫星不稳定性(microsatellite instability,MSI)分析。采用实时荧光定量聚合酶链反应(quantitative real-time polymerase chain reaction,qRT-PCR)检测FBLN1在胃癌样本中的表达情况。结果GEO数据库提示FBLN1在肿瘤组织中表达显著下降(P<0.001),且ROC曲线的AUC为0.89。qRT-PCR证实FBLN1在胃腺癌组织中的表达显著低于癌旁组织。但在胃腺癌患者中,FBLN1表达量与预后呈负相关(P<0.01)。在低度微卫星不稳定(microsatellite low instability,MSI-L)、高度微卫星不稳定(microsatellite high instability,MSI-H)胃癌样本中FBLN1的表达量显著低于微卫星稳定型(microsatellite stability,MSS)。结论FBLN1表达在胃腺癌组织中显著下调,且对胃腺癌有着良好的诊断效能。FBLN1的表达量和MSI负相关。高FBLN1表达水平的胃腺癌患者预后不良。Objective To investigate the differential expression of fibulin-1(FBLN1)in cancer tissues and normal tissues of stomach adenocarcinoma and to reveal its diagnostic and prognostic value.Methods With GEO database(GSE66229 dataset)as the analytical foundation,differential expression analysis,ROC curve plotting were performed towards FBLN1.Survival analysis for FBLN1 was conducted on the TCGA STAD database,and then the relationship of FBLN1 to microsatellite instability(MSI)states was researched.The expression of FBLN1 in gastric sample collected from our hospital was detected with quantitative real-time polymerase chain reaction(qRT-PCR).Results According to the analysis with GEO database,the expression level of FBLN1 was significantly down-regulated in cancer tissues(P<0.001),and the area under the ROC curve was 0.899.qRT-PCR detection demonstrated a much lower level of FBLN1 expression in cancer tissues(1.0±0.20)than that in normal tissue.There was a significantly negative correlation between FBLN1 expression and patient prognosis(P<0.01).The FBLN1 expression of tumor with microsatellite high/low instability(MSI-H/MSI-L)was significantly lower than tumor with microsatellite stability(MSS).Conclusions FBLN1 is decreased in cancer tissues.A high diagnostic efficacy of FBLN1 for stomach adenocarcinoma was observed.There was a significantly negative correlation between FBLN1 expression and MSI states.Patients with higher FBLN1 expression lead to a poorer prognosis.
