可吸收止血纱布联合重组牛碱性成纤维细胞生长因子对腭裂松弛切口的疗效观察

摘　　要：目的观察可吸收止血纱布(商品名:泰绫)联合重组牛碱性成纤维细胞生长因子(rb-bFGF)在腭裂整复术中和术后使用的效果。方法 45例腭裂患儿,随机分为对照组1、对照组2及实验组,各15例。腭裂整复术后,对照组1采用双侧松弛切口填塞可吸收止血纱布;对照组2采用双侧松弛切口填塞rb-bFGF凝胶(商品名:贝复新);实验组采用双侧松弛切口填塞可吸收止血纱布,并填塞rb-bFGF凝胶。比较三组患儿术后松弛切口出血时间以及肉芽组织生长时间。结果实验组术后当日双侧松弛切口出血时间为(2.6±0.6)h,短于对照组2的(3.7±0.4)h,差异具有统计学意义(P<0.05);对照组1的术后当日双侧松弛切口出血时间明显短于对照组2,差异具有统计学意义(P<0.05)。实验组的肉芽组织生长时间为(5.7±0.6)d,短于对照组1的(6.9±0.4)d,差异具有统计学意义(P<0.05);对照组2的肉芽组织生长时间明显短于对照组1,差异具有统计学意义(P<0.05)。结论可吸收止血纱布联合rb-bFGF使用,可以减少手术创面出血,促进手术创面愈合,值得进一步临床观察、研究及推广使用。Objective To observe the effect of absorbable hemostatic gauze(trade name: Tailing) combined with recombinant bovine basic-fibroblast growth factor(rb-bFGF) on the relaxation incision of cleft palate. Methods A total of 45 cases of cleft palate children were randomly divided into control group 1, control group 2 and experimental group, with 15 cases in each group. After cleft palate restoration, absorbable hemostatic gauze was packed to bilateral loose incisions of control group 1, rb-bFGF gel(trade name: Beifuxin) was packed to bilateral loose incisions of control group 2, and absorbable hemostatic gauze and rb-bFGF gel were packed to bilateral loose incisions of experimental group. The postoperative bleeding time of relaxation incision and granulation tissue growth time of the three groups were compared. Results The bleeding time of bilateral relaxation incision of the experimental group was(2.6±0.6) h, which was shorter than(3.7±0.4) h of control group 2, and the difference was statistically significant(P<0.05);the bleeding time of bilateral relaxation incision of control group 2 was obviously shorter than that of control group 2, and the difference was statistically significant(P<0.05). The granulation tissue growth time(5.7±0.6) d of the experimental group was shorter than that of control group 1(6.9±0.4) d, and the difference was statistically significant(P<0.05). The granulation tissue growth time of control group 2 was obviously better than that of control group 1, and the difference was statistically significant(P<0.05). Conclusion Absorbable hemostatic gauze combined with rb-bFGF can reduce wound bleeding of cleft palate surgery and promote surgical wound healing. It is worthy of further clinical observation, research and popularization.
