动静脉联合神经介入溶栓术对缺血性脑血管病患者神经功能及复发的影响

摘　　要：目的探讨动静脉联合神经介入溶栓术对缺血性脑血管病患者的应用效果。方法回顾性分析本院在2017年3月至2019年3月间收治的100例缺血性脑血管病患者的临床资料,按照不同治疗方式,分为对照组(常规治疗+ 静脉溶栓治疗,n = 48)与观察组(常规治疗+ 动静脉联合神经介入溶栓术,n = 52),对比两组患者的神经功能及复发率。结果 出院前1 d 两组患者的美国国立卫生研究院卒中量表(NIH Stroke Scale,NIHSS)评分均比治疗前低,观察组患者的NIHSS 评分比对照组低,差异有统计学意义(P < 0.05);出院后随访半年,观察组复发率为5.77%,明显比对照组的18.75% 低,差异有统计学意义(P < 0.05)。结论 与静脉溶栓治疗相比而言,动静脉联合神经介入溶栓术治疗缺血性脑血管病患者,可显著促进患者神经功能的恢复,并利于降低复发率,减少预后风险。Objective To investigate the effect of arteriovenous combined with nerve interventional thrombolysis on patients with ischemic cerebrovascular disease.Methods The clinical data of 100 patients with ischemic cerebrovascular disease admitted in our hospital from March 2017 to March 2019 were retrospectively analyzed.According to the different treatment methods,the patients were divided into control group(routine treatment+intravenous thrombolysis,n=48)and observation group(routine treatment+arteriovenous combined with nerve interventional thrombolysis,n=52).The neurological function and recurrence rate of the patients in the two groups were compared.Results The scores of National Institutes of Health Stroke Scale(NIHSS)of the two groups 1 d prior to discharge were all lower than those before treatment,and the NIHSS score in the observation group was lower than that in the control group,with statistically significant difference(P<0.05).After 6 months of follow up,the recurrence rate in the observation group was lower than that in the control group(5.77%vs.18.75%),with statistically significant difference(P<0.05).Conclusion Compared with intravenous thrombolytic therapy,arteriovenous nerve interventional thrombolysis in the treatment of patients with ischemic cerebrovascular disease can significantly promote the recovery of nerve function,reduce the recurrence rate,and reduce the risk of prognosis.
