三维放疗联合紫杉醇脂质体在局部中晚期食管癌中的疗效

摘　　要：目的研究三维放疗联合紫杉醇脂质体在局部中晚期食管癌中的临床疗效。方法选取2016年1月至2017年7月收治的局部中晚期食管癌患者84例为对象,将其随机分为A组和B组,每组42例。对应采用三维放疗联合紫杉醇脂质体治疗方式和单纯采用三维放疗治疗方式,比较2组临床疗效、2年内的局控率和生存率及不良反应发生率。结果A组治疗有效率97.62%,较B组(78.57%)高,差异有统计学意义(P<0.05)。A组1年内的局控率和生存率分别为92.86%、78.57%,B组对应的结果为73.81%、54.76%,A组均高于B组,差异有统计学意义(P<0.05)。A组2年内的局控率和生存率分别为85.71%、69.05%,B组对应的结果为64.29%、52.86%,A组均高于B组,差异有统计学意义(P<0.05)。2组治疗后肿瘤直径均小于治疗前,血小板计数和生存质量均高于治疗前,差异有统计学意义(P<0.05)。A组治疗后肿瘤直径[(2.05±1.11)cm]较B组[(3.11±1.54)cm]短,血小板计数[(135.21±22.75)10^9/L]较B组[(111.53±17.86)10^9/L]高,生存质量[(77.87±12.32)分]较B组[(69.87±12.21)分]高,差异有统计学意义(P<0.05)。A组恶性呕吐、肌功能异常、肝功能异常、血小板减少、白细胞减少的发生率分别为42.86%、11.90%、7.14%、28.57%、35.71%,B组对应的结果分别为69.05%、33.33%、26.19%、54.76%、61.90%,A组均较B组低,差异有统计学意义(P<0.05)。结论在对局部中晚期食管癌患者展开治疗的过程中,在三维放疗的基础上联合紫杉醇脂质体进行治疗,其不仅具有较为显著的临床治疗效果,且在局控率和生存率方面也较高,同时还具有较高的安全性,因而值得在临床治疗中给予应用和推广。Objective To investigate the therapeutic effects of three-dimensional radiotherapy combined with paclitaxel liposomes on local middle and advanced stage esophageal cancer.Methods A total of 84 patients with local middle and advanced stage esophageal cancer who were treated in our hospital from January 2016 to July 2017 were enrolled in the study,who were randomly divided into group A and group B,with 42 cases in each group.The patients in group A were treated by three dimensional radiotherapy combined paclitaxel liposome,however,the patients in control group were treated by three dimensional radiotherapy only.The clinical curative effects 2-year local control rate and survival rate as well as the incidence of adverse reactions were observed and compared between the two groups.Results The effective rate in group A was significantly higher than that in group B(P<0.05).The 1-year local control rate and survival rate in group A were significantly higher than those in group B(P<0.05).The 2-year local control rate and survival rate in group A were significantly higher than those in group B(P<0.05).moreover the tumor diameters in both groups after treatment were significantly smaller than those before treatment,and the platelet count and quality of life were significantly higher than those before treatment(P<0.05).After treatment,the tumor diameter in group A was significantly smaller than that in group B,and the platelet count anfd life quality scores in group A were significantly higher than those in group B(P<0.05).In addition the incidence rates of malignant vomiting,abnormal muscle function,abnormal liver function,thrombocytopenia and leukopenia in group A were significantly lower than those in group B(P<0.05).Conclusion The three-dimensional radiotherapy combined with paclitaxel liposomes in treatment of local middle and advanced stage esophageal cancer can obtain significant therapeutic effects,which can improve local control rate and survival rate,with better safety,therefor,it is worthy of application and popul
