广泛性焦虑量表在心血管危重症患者焦虑情绪筛查中的应用

摘　　要：目的:探讨中文版广泛性焦虑量表(the generalized anxiety disorder 7-item scale,GAD-7)在心血管危重症患者中筛查焦虑情绪的信度和效度,并确定其最佳筛查临界值。方法:选取2018年9月—2019年1月入住南通大学附属医院心脏病监护病房的121例患者为研究对象,采用中文版GAD-7及焦虑自评量表(self rating anxiety scale,SAS)对患者进行测定,以信度分析、探索性因子分析、受试者工作特征(receiver operating characteristic,ROC)曲线来评估GAD-7的信效度。结果:GAD-7量表内部一致性Cronbach's a系数为0.912,各条目与量表总分的相关系数为0.712~0.839(P<0.01);验证性因子分析显示该量表为一因子结构,解释方差65.82%;GAD-7分界值为8分时,灵敏度和特异度最佳划界分为90.0%、84.7%,GAD-7的ROC曲线下面积(area under curve,AUC)为0.952(SE=0.024,95%CI:0.906~0.998)。结论:GAD-7用于评估心血管危重症患者焦虑情绪有良好的信度和效度,可在心血管危重症患者中推广应用。Objective:To evaluate the reliability and validity of a Chinese version of the generalized anxiety disorder 7-item scale(GAD-7)in screening out anxiety disorders,and to determine optimal cutoff value among patients with critical cardiovascular disease.Methods:Totally 121 patients were selected from Cardiac Intensive Care Unit from September 2018 to January 2019.The patients were measured by Chinese version of GAD-7 and self-rating anxiety scale(SAS).Internal consistency,explosive factor analysis,receiver operating characteristic(ROC)curve were used to evaluate the reliability and validity of GAD-7.Results:The Cronbach's a coefficient of GAD-7 was 0.912,and the range of correlation coefficient within the 7 items and between the total score of the scale was 0.712-0.839(P<0.01).The scale was shown to be one dimensional through confirmatory factor analysis,which explained 65.82%of total variance.A cut point of 8 showed adequate values of the sensitivity and specificity for GAD-7 was 90.0%,84.7%,respectively,and the area under the curve(AUC)was 0.952(SE=0.024,95%CI:0.906-0.998).Conclusion:The Chinese version GAD-7 is a reliable and valid measure to screen anxiety,which was worth to use in patients with critical cardiovascular disease.
