摘  要:目的探讨信号素分子3G(SEMA3G)在涎腺腺样囊性癌(SACC)组织中的表达及基因启动子甲基化水平,以及与患者临床病理特征和预后的关系。方法选取2002年2月至2013年12月中国科学院大学附属肿瘤医院(浙江省肿瘤医院)收治的SACC患者55例。采用免疫组化SP法检测55例SACC组织及对应癌旁正常组织中SEMA3G的表达情况,采用实时荧光定量甲基化特异性聚合酶链反应检测SEMA3G基因的甲基化情况。分析SACC组织SEMA3G表达水平、基因甲基化水平与临床病理特征及预后的关系。结果SACC组织SEMA3G表达水平低于癌旁正常组织,甲基化水平高于癌旁正常组织(均P<0.05);SEMA3G表达水平与SACC患者是否复发、T分级及TNM分期有关(均P<0.05)。SEMA3G基因甲基化水平与SACC患者T分级、TNM分期有关(均P<0.05)。两者与患者性别、年龄、肿瘤位置、组织类型、是否侵犯邻近组织、淋巴结转移、神经累及、肌层浸润及远处转移均无关(均P>0.05)。与阳性表达患者比较,SEMA3G阴性表达的SACC患者预后较差(P<0.05)。结论SEMA3G异常表达与SACC发生、预后可能存在密切关系,启动子区甲基化可能是该基因表达失活的主要调控机制之一。Objective To investigate the expression of SEMA3G and the level of promoter methylation of SEMA3G gene in salivary adenoid cystic carcinoma(SACC),and its relation with the clinicopathologic feature and prognosis of patients.Methods The expression of SEMA3G was examined with immunohistochemistry(SP method)in SACC tissues and adjacent normal tissues of 55 patients with SACC.Quantitative methylation-specific PCR(qMSP)was used to detect the promoter methylation status of SEMA3G gene in the above samples.The relationship of the level of SEMA3G expression and SEMA3G gene methylation with clinicopathologic feature and prognosis of SACC patients were analyzed.Results The expression of SEMA3G was down-regulated in SACC tissues compared to adjacent normal tissues;the promoter methylation level of SEMA3G gene was up-regulated in SACC tissues compared to adjacent normal tissues(P<0.05).The expression of SEMA3G was associated with recrudescence,T-staging and TNM(P<0.05).The promoter methylation level of SEMA3G gene was associated with T-staging and TNM(P<0.05).While both had no relationship with gender,age,histotype,adjacent tissues invasion,lymph node metastasis,nerve invasion,muscle invasion and distant metastasis(P>0.05).SACC patients with negative expression of SEMA3G had a worse prognosis compared to those with positive expression of SEMA3G(P<0.05).Conclusion The aberrant expression of SEMA3G might be associated with the tumor progression,prognosis of SACC,and promoter methylation may be one of the main regulatory mechanisms for inactivation of SEMA3G expression.
