CT引导下脊神经脉冲射频调控术治疗带状疱疹性神经病理性疼痛的效果及安全性分析

摘　　要：目的探究疱疹性神经病理性疼痛采用CT引导下脊神经脉冲射频调控术治疗的效果以及安全性。方法回顾性分析2016年9月至2018年3月我院收治90例疱疹性神经病理性疼痛患者相关资料,依据治疗方式不同分为药物组(n=40)与观察组(n=50),药物组患者接受CT引导下经椎间孔药物(臭氧+消炎镇痛液)注射术治疗,观察组患者接受CT引导下脊神经脉冲射频调控术治疗。比较两组患者治疗前后相关疼痛评分、神经功能评分以及血清炎症因子水平、治疗效果、不良反应发生率。结果两组患者治疗后VAS评分、RPI评分以及PPI评分显著下降(P<0.05),治疗后观察组患者上述疼痛评分明显低于药物组患者(P<0.05);治疗后两组患者改良Barthel指数与IL-10水平较治疗前上升,NIHSS评分与TNF-α水平较治疗前下降,差异比较具有统计学意义(P<0.05),治疗后观察组患者改良Barthel指数与IL-10水平明显高于药物组,NIHSS评分与TNF-α水平显著低于药物组(P<0.05);观察组患者治疗总有效率高于药物组患者,差异比较具有统计学意义(94.0%VS70.0%,P<0.05);观察组与药物组患者不良反应发生率分别为12.00%与15.00%,两组发生率比较无统计学意义(P>0.05)。结论疱疹性神经病理性疼痛治疗应用脊神经脉冲射频调控术可有效减轻患者的疼痛感受,对患者神经功能影响小,安全性较高。Objective To investigate the effects and safety of CT-guided spinal nerve pulse radiofrequency regulation technology in the treatment of neuropathic pain caused by herpes zoster.Methods The relevant treatment data of 90 patients with neuropathic pain caused by herpes zoster who were admitted to the hospital from September 2016 to March 2018 were analyzed retrospectively.The patients were divided into the drug group(n=40)and the observation group(n=50)according to different treatment methods.Patients in the drug group were treated with CT-guided drug injection(ozone and antiinflammatory analgesic liquid)via intervertebral foramen while patients in the observation group were treated with CT-guided spinal nerve pulse radiofrequency regulation technology.Pain scores,neurological function scores,levels of serum inflammatory factors,treatment effects,and adverse reaction rates were compared between the two groups.Results VAS scores,RPI scores and PPI scores of both groups were significantly decreased after treatment(P<0.05).The above pain scores of the observation group were significantly lower than those of the drug group(P<0.05).Modified Barthel index and IL-10 levels in both groups were increased while NIHSS scores and TNF-αlevels were decreased after treatment(P<0.05).After treatment,the modified Barthel index and IL-10 level in the observation group were higher than those in the drug group,while NIHSS score and TNF-αlevel were significantly lowerthan the drug group(P<0.05).The total response rate of the observation group was higher than that of the drug group(94.0%VS 70.0%,P<0.05).The incidence rates of adverse reactions in the observation group and the drug group were 12.00%and 15.00%,no significant difference(P>0.05).Conclusion The application of spinal nerve pulse radiofrequency regulation technology in the treatment of neuropathic pain caused by herpes zoster can effectively alleviate the pain of patients,with little impact on patients'neurological function and high safety.
