双歧杆菌四联活菌片联合安肠消痞汤对腹泻型肠易激综合征与功能性消化不良重叠患者胃肠激素水平的影响

摘　　要：目的探讨双歧杆菌四联活菌片联合安肠消痞汤对腹泻型肠易激综合征(D-IBS)和功能性消化不良(FD)重叠患者胃肠激素水平的影响。方法选取2017年1月至2019年2月我院内科门诊治疗的76例D-IBS与FD重叠患者为研究对象,随机分为两组。对照组患者口服双歧杆菌四联活菌片1.5 g/次,3次/d,温开水服用。观察组患者在对照组基础上加用安肠消痞汤治疗,1剂/d。两组患者连用6周。观察两组患者治疗前后胃肠激素水平的变化,并比较临床效果及不良反应。结果治疗6周后,两组患者血清生长抑素和血管活性肠肽水平较治疗前明显下降,血清胃动素和促胃液素水平较治疗前明显上升,且治疗后观察组变化幅度大于对照组(均P<0.05)。观察组患者临床总有效率明显高于对照组(94.74%vs 78.95%,χ2=4.151,P=0.044)。观察组与对照组治疗中分别出现不良反应3例(7.89%)和5例(13.16%),症状较轻微,两组比较差异无统计学意义(χ2=0.142,P=0.734)。结论双歧杆菌四联活菌片联合安肠消痞汤治疗D-IBS与FD重叠患者的效果确切,安全性较高,其作用可能与其能调节胃肠激素紊乱,下调血清SS和VIP水平,上调血清MOT和GAS水平,改善胃肠道功能密切相关。Objective To discuss the influence of Bifidobacterium(live)Tetravaccine Tablets combined with Anchang-Xiaopi Decoction on gastrointestinal hormone levels of patients with overlapped diarrhea-predominant irritable bowel syndrome(D-IBS)and functional dyspepsia(FD).Methods A total of 76 patients with overlapped D-IBS and FD,who were given medical treatment in Department of Medicine from January 2017 to February 2019,were divided into two groups at random.All the patients were given Bifidobacterium(live tetravaccine tablets)1.5 g tid orally,while those in observation group were additionally given Anchang-Xiaopi Decoction one dose per day,for 6 weeks.The changes of gastrointestinal hormone levels of patients in both groups were observed before and after treatment,and the clinical efficacy and untoward effects were compared between groups.Results After treatment,the serum SS and VIP levels of patients in both groups obviously declined,while the serum MOT and GAS levels obviously rose,with the observation group superior to the control group(all P<0.05).Meanwhile,the total clinical efficiency rate in observation group was much higher than that in control group(94.74%vs 78.95%,χ~2=4.151,P=0.044).3 and 5 cases of untoward effects appeared in observation group(7.89%)and control group(13.16%)respectively during the medical treatment with mild symptoms(χ~2=0.142,P=0.734).Conclusion Bifidobacterium(live)Tetravaccine Tablets combined with Anchang-Xiaopi Decoction has reliable clinical efficacy on patients with overlapped D-IBS and FD,with high security,the mechanism of action of which may have close relation with its effect in adjusting the disorder of gastrointestinal hormone,lowering the serum SS and VIP levels,raising the serum MOT and GAS levels and improving the gastroenteric function.
