超声心动图诊断左心室室壁瘤的临床应用

摘　　要：目的探讨超声心动图对左心室室壁瘤的临床诊断价值。方法选取我院近12年超声心动图诊断左心室室壁瘤患者43例,超声采用多切面、多角度观察心脏形态、左心室室壁有无运动减低、无运动或反常运动,并描述室壁瘤发生的部位、形态、大小等。结果超声心动图诊断左心室室壁瘤43例,解剖性真性室壁瘤40例,功能性真性室壁瘤3例,43例均发生在心尖部,其中累及前壁及前间隔37例,前间隔2例,前侧壁2例,下壁2例。左室舒张末内径(55.0±8.3)mm、射血分数(45.0±9.8)%、室壁瘤直径(37.9±10.8)mm。结论超声心动图是诊断左心室室壁瘤和评价临床病情及预后的可靠手段,具有无创、方便、快捷的优势。Objective To investigate the diagnostic value of echocardiography(UCG)for left ventricular aneurysm(LVA).Methods Forty-three patients with left ventricular aneurysm diagnosed by echocardiography in recent 12 years in our hospital were selected.Ultrasound was used to observe the shape of the heart with multi-slice and multi-angle to observe whether there were hypokinesis or paradoxical movement,in the left ventricular wall,and to describe the location,shape,size where the ventricular aneurysm occurred.Results 43 cases were diagnosed left ventricular aneurysm by echocardiography,Including anatomic true ventricular aneurysm(40 cases),functional true ventricular aneurysm(3 cases).All of them occurred in the apex of the heart,involving the anterior and|anteroseptal wall(37 cases),anteroseptal(2 cases),anterior lateral wall(2 cases),inferior wall(2 cases).Left ventricular end diastolic diameter was(55.0±8.3)mm,ejection fraction was(45.0±9.8)%,and diameter of ventricular aneurysm was(37.9±10.8)mm.Conclusions Echocardiography is a reliable method to diagnose the left ventricular aneurysm and evaluate the clinical condition and prognosis.
