氧化锆全瓷冠应用不同粘接剂对粘接强度与边缘微渗漏的影响

摘　　要：目的将不同粘接剂用于氧化锆全瓷冠中,分析其对粘接强度及边缘微渗漏的影响。方法选取2016年8月-2017年9月收治的80例因牙周炎拔除的无龋损磨牙患者(80颗)分组研究,根据粘接剂不同分为1组(Opitbond Versa+Maxcem Elite)、2组(RelyX Unicem)、3组(CX)、4组(Maxcem Elite),4组于制作氧化锆全瓷冠后分别使用不同类型的粘接剂,对各组粘接强度、边缘微渗漏情况进行比较分析。结果 1、2、4 3组各组间微渗漏值差异无统计学意义(P>0.05),1组(398.65±12.37)μm、2组(401.37±12.25)μm、4组(402.43±12.19)μm微渗漏值与3组(431.52±16.78)μm相比,均相对更低,差异有统计学意义(P<0.05);1、2、4组粘接强度与3组相比,均相对更高,差异有统计学意义(P<0.05),2、4组粘接强度相比,差异无统计学意义(P>0.05),除此之外,各组两两相比,粘接强度均,差异有统计学意义(P<0.05),其中1组粘接强度最高,3组粘接强度最低。结论对氧化锆全瓷冠而言,不同粘接剂效果存在差异,其中树脂类粘接剂性能相对较好,用于氧化锆全瓷冠中可增强粘接强度,减少冠边缘微渗漏程度,但今后仍需大样本实验及长时间临床观察进一步证实其临床效果。Objective To apply different adhesives to zirconia all-ceramic crowns and analyze their effects on the bonding strength and edge leakage.Methods A group study of 80 patients with caries-free molars(80 teeth)who were removed from periodontitis from August 2016 to September 2017 were selected and divided into group 1(Opitbond Versa+Maxcem Elite)according to different adhesives;group 2(RelyX Unicem),group 3(CX),group 4(Maxcem Elite),the four groups use different types of adhesives after the zirconia allceramic crown is made,and the bonding strength and edge of each group are slightly leaked.The situation is compared and analyzed.Results There was no significant difference in the microleakage value among the three groups of 1,2,and 4(P>0.05),group 1(398.65±12.37)μm,group 2(401.37±12.25)μm,and group 4(402.43±12.19)μm,compared with the group 3(431.52±16.78)μm,the values were relatively lower and statistically different(P<0.05);compared with the group 3,the bonding strength of the groups 1,2 and 4 were relatively higher,with statistically difference(P<0.05).Compared with the bonding strength of the groups 2 and 4,the difference was not significant(P>0.05).In addition,the bonding strength of each group was statistically different(P<0.05),one group has the highest bonding strength,and the group 3 has the lowest bonding strength.Conclusion For zirconia all-ceramic crowns,there are differences in the effects of different adhesives.Among them,resin-based adhesives have relatively good performance.Used in zirconia all-ceramic crowns can enhance the bonding strength and reduce the degree of microleakage at the edge of the crown.However,large sample experiments and long-term clinical observation are still needed to further confirm its clinical effects.
