经导管肝动脉栓塞化疗联合射频消融术在中晚期肝癌患者中的应用研究

摘　　要：目的探讨经导管肝动脉栓塞化疗(transcatheter arterial chemoembolization,TACE)联合射频消融术(radiofrequency ablation,RFA)在中晚期肝癌患者中的应用效果及安全性。方法回顾性分析广东省惠州市中心人民医院2016年5月至2018年7月102例中晚期肝癌患者临床资料,根据治疗方式分为对照组(52例)和观察组(50例),对照组采用TACE治疗,观察组采用TACE联合RFA治疗。对比两组肿瘤坏死情况、治疗效果、血清指标及预后。结果治疗后1个月,观察组肿瘤完全坏死率(58.0%)明显高于对照组(36.5%)(P<0.05)。治疗后1个月,观察组总体治疗效果明显好于对照组(P<0.05)。治疗后1个月,观察组血清甲胎蛋白(alpha fetoprotein,AFP)、总胆红素(total bilirubin,TBIL)、谷丙转氨酶(alanine aminotransferase,ALT)均明显低于对照组,两组比较差异有显著性(P<0.05)。观察组1年、2年复发率明显低于对照组(P<0.05);观察组1年、2年生存率明显高于对照组,两组比较差异有显著性(P<0.05)。结论TACE联合RFA能够明显提高中晚期肝癌患者肿瘤坏死率,提高治疗效果,改善患者血清指标及预后。Objective To investigate the effect and safety of transcatheter hepatic arterial embolization chemotherapy(TACE)combined with percutaneous radiofrequency ablation(RFA)in patients with advanced liver cancer.Method The clinical data of 102 patients with advanced liver cancer in our hospital from May 2016 to July 2018 were retrospectively analysised;According to the treatment mode,the patients were divided into control group(52 cases)and observation group(50 cases).The control group was treated with TACE,while the observation group was treated with TACE combined with RFA.The tumor necrosis situation,treatment effect,serum index and prognosis were compared between the two groups.Result One month after treatment,the tumor necrosis rate(58.0%)in the observation group was significantly higher than that in the control group(36.5%),there was significant difference between the two groups(P<0.05).One month after treatment,the overall treatment effect in the observation group was significantly better than that in the control group,there was significant difference between the two groups(P<0.05).One month after treatment,the AFP,TBIL and ALT in the observation group were significantly lower than those in the control group(P<0.05).The annual recurrence rate of observation group 1 year and 2 years were significantly lower than that of the control group(P<0.05);the annual survival rates of observation group 1 year and 2 years were significantly higher than the control group(P<0.05).Conclusion TACE combined with RFA could significantly increase the tumor necrosis rate of patients with intermediate and advance liver cancer,and could improve the treatment effect,serum index and prognosis of patients.
