试论术中超声引导下导丝定位联合体表定位切除触诊阴性乳腺病灶  

On Intraoperative Ultrasound-guided Guide Wire Positioning Combined with Body Surface Positioning to Remove Palpable Negative Breast Lesions

摘　　要：目的研究术中超声引导下导丝定位联合体表定位在切除触诊阴性乳腺病灶中的作用。方法选取该院自2018年7月—2019年7月,收治的触诊阴性的乳腺病患者共106例,将患者随机分为观察组和对照组,对照组采取药物治疗,观察组采取术中超声引导下导丝定位联合体表定位切除病灶,对比两组患者的治疗效果,分析观察组的基本信息。结果观察组、对照组的总有效率分别为98.1%、77.4%(χ2=10.609,P<0.05);观察组中,有患者53例,病灶数为92个,定位操作时间在2~5 min内,平均耗时3 min,患者没有出现感染、导丝移位等并发症,所有乳腺良性肿瘤均行肿物切除术,恶性肿瘤根据患者实际情况选择保乳手术或是乳腺切除。患者在术后没有出现感染、血肿等并发症,没有病灶残留。结论在触诊阴性的乳腺病患者中,行术中超声引导下导丝定位联合体表定位切除病灶,可以有效切除患者的病灶,最大限度的保留乳房的外形,切除范围较为精准,引发的并发症少,患者疼痛程度低。Objective To study the role of intraoperative ultrasound-guided guide wire positioning combined with body surface positioning in the removal of palpable negative breast lesions.Methods A total of 106 patients with palpation-negative breast disease from July 2018 to July 2019 were randomly divided into an observation group and a control group.The control group was treated with drugs and the observation group was treated with surgery.Guide wire positioning combined with body surface positioning under the guidance of mid-ultrasound to remove the lesion,compare the treatment effect of the two groups of patients,and analyze the basic information of the observation group.Results The total effective rates of the observation group and the control group were 98.1%and 77.4%,respectively,(χ2=10.609,P<0.05);there were 53 patients in the observation group with 92 lesions,and the positioning operation time was 2-5minutes,and it took an average of 3 minutes.The patient did not have complications such as infection and guide wire displacement.All benign breast tumors underwent tumor resection.Malignant tumors were selected according to the actual situation of the patient.The patient had no complications such as infection and hematoma after the operation,and no residual disease.Conclusion In patients with breast disease with negative palpation,intraoperative ultrasound-guided guide wire positioning combined with body surface positioning to remove the lesion can effectively remove the patient's lesion,maximize the preservation of the shape of the breast,and the resection range is more accurate.There are few complications and the patient's pain level is low.
