蟾酥注射液联合阿帕替尼治疗晚期胃癌患者的疗效

摘　　要：目的观察蟾酥注射液液联合阿帕替尼治疗晚期胃癌患者的疗效。方法确诊为晚期胃癌(Ⅳ期)经二线及二线治疗失败后接受阿帕替尼治疗的患者50例为研究对象,随机分为观察组和对照组,每组均为25例。对照组给予阿帕替尼单药口服,观察组在阿帕替尼治疗的基础上给予蟾酥注射液。治疗4个疗程后根据影像学结果评价两组患者的治疗效果,记录不良反应的发生情况,并检测患者外周血肿瘤标志物、免疫指标情况。结果(1)观察组总有效率(ORR)为20%,对照组ORR为8%,差异无统计学意义(P=0.221)。观察组疾病控制率(DCR)为76%,对照组DCR为48%,差异有统计学意义(P=0.041);(2)观察组毒副反应发生率低于对照组(P<0.05);(3)两组治疗前肿瘤标志物(CA199、CEA)水平无明显差异,用药后均明显下降,观察组优于对照组(P<0.05);(4)用药后观察组的免疫指标(CD3+、CD4+、CD8+、CD4+/CD8+、IgG、IgA、IgM)变化水平优于对照组(均P<0.05)。结论蟾酥注射液可改善晚期胃癌阿帕替尼的治疗效果。Objective To observe the efficacy of toad venom injection combined with apatinib in patients with advanced gastric cancer.Methods Fifty patients diagnosed with advanced gastric cancer(stageⅣ)who were treated with apatinib after second-line and second-line treatment failure were randomly divided into the test group and control group with 25 patients in each group.The control group was administered with apatinib orally,while the test group was with toad venom injection on the basis of apatinib treatment.After 4 courses of treatment,the treatment effect of the two groups of patients was evaluated according to the imaging results,the occurrence of adverse reactions was recorded,and peripheral blood tumor markers and immune indicators were detected.Results The total effective rate(ORR)was 20%in the test group and 8%in the control group(P=0.221).The disease control rate(DCR)was 76%in the study group and 48%in the control group(P=0.041).The incidence of toxic side effects in the test group was lower than that in the control group(P<0.05).There was no significant difference in the levels of tumor markers(CA199,CEA)between the two groups before medication,but the levels decreased significantly after medication,and the test group was better than the control group(P<0.05).The changes of immune indexes(CD3+,CD4+,CD8+,CD4+/CD8+,IgG,IgA,IgM)in the test group were better than those in the control group(P<0.05).Conclusion Toad venom injection can improve the treatment effect of apa-tinib in advanced gastric cancer.
