摘 要:目的探讨浸润性乳腺癌中的超声影像表现及其对患者临床特征和肿瘤标志物表达的影响。方法对78例浸润性乳腺癌患者进行超声检查,所有患者均经手术或穿刺活检确诊,比较不同临床特征浸润性乳腺癌患者的超声成像评分;免疫组化法检测78例浸润性乳腺癌患者的肿瘤标志物表达情况,包括雌激素受体(ER)、人表皮生长因子受体2(HER2)。结果超声成像评分结果显示,78例浸润性乳腺癌患者中3分9例,4分33例,5分36例。超声成像评分可能与组织学分级、病理类型、淋巴结转移情况和脉管侵犯情况有关,差异均有统计学意义(P﹤0.05)。免疫组化结果显示,浸润性乳腺癌患者乳腺癌组织中ER阳性62例,阴性16例;HER2阳性18例,阴性60例。浸润性乳腺癌患者乳腺癌组织中ER阳性表达率可能与边缘毛刺、恶性晕环和微钙化有关,HER2阳性表达率可能与微钙化有关,差异均有统计学意义(P﹤0.05)。结论浸润性乳腺癌的超声表现与临床特征和肿瘤标志物的表达情况可能有关,对预测浸润性乳腺癌生物学特性具有一定的临床价值。Objective To explore the ultrasound imaging performance in invasive breast cancer and its influence on the clinical characteristics and tumor marker expression.Method A total of 78 patients with invasive breast cancer confirmed by surgery or puncture biopsy were selected and scanned with ultrasonography.The ultrasound imaging scores from invasive breast cancer patients with varying clinical features were compared.The immunohistochemical staining assay was used to detect the expression of tumor markers like estrogen receptor(ER)and human epidermal growth factor receptor 2(HER2).Result The ultrasonography result indicated that 9 cases were scored 3,33 cases were scored 4,and 36 cases were scored 5 in 78 patients with invasive breast cancer.The imaging results may be related to factors like histological grade,pathological type,lymph node metastasis and vascular invasion,and the differences were statistically significant(P<0.05).The results of immunohistochemistry staining assay showed that 62 cases were ER positive and 16 cases were negative;18 cases were HER2 positive and 60 cases were negative in patients with invasive breast cancer.The positive expression rate of ER in breast cancer tissues of patients with invasive breast cancer may be related to margirin burr,malignant halo and microcalcification,and the positive expression rate of HER2 may be related to microcalcification,and the differences were statistically significant(P<0.05).Conclusion Ultrasound manifestations of invasive breast cancer are closely related to clinical features and expression level of classic tumor markers,and have some clinical value in the prediction of the biological characteristics of invasive breast cancer.
