摘　　要：目的探讨贞芪扶正胶囊联合吉西他滨和顺铂(GP)化疗方案对晚期非小细胞肺癌(NSCLC)患者的临床疗效及对血清神经纤毛蛋白1(NRP-l)、B7-H3水平的影响。方法根据随机数字表法将62例晚期NSCLC患者随机分为对照组和观察组,每组31例。对照组采用GP化疗方案,观察组在此基础上加用贞芪扶正胶囊。比较两组患者的临床疗效、免疫功能指标(CD3+、CD4+和CD4+/CD8+)、血清NRP-l水平、B7-H3水平和不良反应发生情况。结果观察组患者的有效率为48.39%,高于对照组的22.58%(P﹤0.05)。治疗后,观察组患者的CD3+、CD4+水平和CD4+/CD8+均高于对照组患者(P﹤0.05)。治疗后,两组患者的血清NRP-l、B7-H3水平均低于本组治疗前,且观察组患者的血清NRP-l、B7-H3水平均低于对照组患者(P﹤0.05)。观察组患者胃肠道反应、肝肾功能损害、心脏毒性和白细胞减少的发生率均低于对照组患者(P﹤0.05)。结论贞芪扶正胶囊联合GP化疗方案治疗晚期NSCLC疗效显著,能够增强患者的免疫功能,降低血清NRP-l、B7-H3水平,且不良反应少。Objective To investigate the clinical efficacy of Zhenqi Fuzheng capsule combined gemcitabine+cisplatin(GP)chemotherapy in patients with advanced non-small cell lung cancer(NSCLC)and its effect on the expression of serum neuropilin-1(NRP-1)and B7-H3.Method 62 patients with advanced NSCLC were randomly divided into the control group and observation group(each n=31),treated with GP chemotherapy and GP chemotherapy+Zhenqi Fuzheng capsule,resptctively.The clinical efficacy,immune function(CD3+,CD4+and CD4+/CD8+),serum NRP-1 and B7-H3 expression levels and adverse reactions were compared between the two groups.Result The effective rate in the observation group was significantly higher than that in the control group(48.39%vs 22.58%,P<0.05).The CD3+,CD4+and CD4+/CD8+in the observation group were significantly higher than those in the control group after treatment(P<0.05).The expression levels of serum NRP-1 and B7-H3 in both groups after treatment were significantly lower than those before treatment,with significantly lower levels in observation group than those in the control group(P<0.05).The incidence rate of gastrointestinal reactions,hepatic and renal impairment,cardic toxicity and leucocytopenia in the observation group was lower than that in the control group during treatment(P<0.05).Conclusion Zhenqi Fuzheng capsule combined GP chemotherapy is effective in the treatment of advanced NSCLC,which can enhance the immune function and reduce the levels of serum NRP-1 and B7-H3 with less adverse reactions.
