摘　　要：目的探讨腹腔镜微创肿瘤细胞减灭术治疗晚期卵巢癌患者的临床应用价值。方法将80例晚期卵巢癌患者按治疗方式不同分为腔镜组37例及传统组43例,腔镜组采用腹腔镜肿瘤细胞减灭术治疗,传统组采用传统开腹手术治疗。比较两组患者的手术创伤指标、手术前后T淋巴细胞水平、并发症发生率、术后2年生存情况。结果腔镜组患者术中出血量明显少于传统组,术后肛门首次排气时间、住院时间均明显短于传统组,差异均有统计学意义(P﹤0.01)。术后24 h,腔镜组患者的外周血中CD3+、CD4+/CD8+水平均高于传统组,差异均有统计学意义(P﹤0.05)。腔镜组患者并发症发生率低于传统组,差异有统计学意义(P﹤0.05)。两组患者的2年生存率及中位生存时间比较,差异均无统计学意义(P﹥0.05)。结论腹腔镜微创肿瘤细胞减灭术治疗晚期卵巢癌患者的效果与开腹手术差异不大,但是前者具有手术创伤小、对患者免疫功能影响小、并发症相对较少的优势。Objective To explore the clinical value of laparoscopic minimally invasive tumor cytoreductive surgery in the treatment of patients with advanced ovarian cancer.Method 80 patients with advanced ovarian cancer were divided into the laparoscopic group(n=37)and conventional group(n=43),treated with laparoscopic cytoreductive surgery and conventional laparotomy,respectively.The surgical trauma indexes,pre-and post-operative T lymphocytes,complications,and 2-year survival rate after operation were compared.Result The intraoperative blood loss,the first postoperative anus exhaust time,and the time of hospital stay in the laparoscopic group were significantly lower than those in the conventional group(P<0.01).The peripheral blood CD3+and CD4+/CD8+24 h after operation in the laparoscopic group were significantly higher than those in the conventional group(P<0.05),with significantly lower complication rate than that in the conventional group(P<0.05).There was no significant difference for the 2-year survival rate and 2-year median survival time between the two groups(P>0.05).Conclusion There is no significant difference for the effect of laparoscopic minimally invasive tumor cytoreductive surgery compared with the laparotomy in the treatment of patients with advanced ovarian cancer,but with minimal surgical trauma,less impact on the immune function,and relatively less complications.
