摘 要:目的探讨低剂量CT扫描与超声造影在诊断甲状腺结节中的应用价值。方法选择本院诊治的118例甲状腺结节患者,经病理诊断为恶性甲状腺结节40例(恶性组)、良性甲状腺结节78例(良性组)。所有患者均给予低剂量CT扫描与超声造影诊断。记录成像特征并判断诊断价值。结果恶性组的CT增强模式以不均匀增强为主,良性组以环状增强为主,两组的CT增强模式比较,差异具有统计学意义(P<0.05)。两组的超声回声、边界、钙化、声晕情况比较,差异具有统计学意义(P<0.05)。恶性组的AUC与MTT均显著低于良性组(P<0.05)。低剂量CT扫描与超声造影诊断甲状腺结节的灵敏度分别为97.50%、100.00%,特异度分别为96.15%、98.72%。结论低剂量CT扫描与超声造影均能清晰地定量与定性显示甲状腺结节的特征,也能明确微血管灌注情况,均有较好的应用价值。Objective To explore the application value of low-dose CT scanning and contrast-enhanced ultrasound in the diagnosis of thyroid nodules.Methods A total of 118 patients with thyroid nodules diagnosed and treated in our hospital were selected,40 cases were malignant thyroid nodules(malignant group)and 78 cases were benign thyroid nodules(benign group)by pathologic diagnosis.All patients were diagnosed with low-dose CT scanning and contrast-enhanced ultrasound.The imaging characteristics were recorded,and the diagnostic value were judged.Results The CT enhancement mode of the malignant group was mainly uneven enhancement,while that of the benign group was mainly circular enhancement.The difference of CT enhancement mode between the two groups was statistically significant(P<0.05).There were significant differences in ultrasonic echo,boundary,calcification and halo between the two groups(P<0.05).AUC and MTT of the malignant group were significantly lower than those of the benign group(P<0.05).The sensitivity of low-dose CT scanning and contrast-enhanced ultrasound in the diagnosis of thyroid nodules were 97.50%and 100.00%respectively,and the specificity were 96.15%and 98.72%,respectively.Conclusion Low-dose CT scanning and contrast-enhanced ultrasound can clearly show the characteristics of thyroid nodules quantitatively and qualitatively,and can also clarify the microvascular perfusion,which have good application value.
