不同促排卵方案对宫腔内人工授精妊娠结局的影响

摘　　要：目的分析不同的促排卵方案在宫腔内人工授精中的应用效果。方法选取2018年1月至2019年1月我院收治的120例不孕患者作为研究对象,根据不同的治疗方案将其分为来曲唑组、来曲唑+注射用尿促性素(HMG)组、来曲唑+丽申宝组,各40例。比较三组的治疗效果。结果三组的HCG日优势卵泡数目、子宫内膜厚度比较,差异有统计学意义,且来曲唑+丽申宝组明显优于其他两组(P<0.05);三组的HCG日优势卵泡大小无显著差异(P>0.05)。三组的流产率、异位妊娠率、多胎妊娠率无显著差异(P>0.05);三组的临床妊娠率比较,差异有统计学意义,且来曲唑+丽申宝组高于其他两组(P<0.05)。结论来曲唑联合丽申宝的促排卵效果较好,可能与药物作用于人体后促进成熟卵泡排出多有关,且成熟的卵泡又促进了药物的作用,明显提高了临床妊娠率。Objective To analyze the application effects of different ovulation induction schemes on intrauterine artificial insemination.Methods A total of 120 infertile patients admitted in our hospital from January 2018 to January 2019 were selected as study objects and divided into the Letrozole group,the Letrozole+HMG group and the Letrozole+uFSH-HP group according to different treatment regimens,with 40 cases in each group.The therapeutic effects of the three groups were compared.Results The number of dominant follicles and endometrial thickness on the day of HCG injection among the three groups were statistically significant,and those in the Letrozole+uFSH-HP group were significantly better than the other two groups(P<0.05);there was no significant difference in the size of dominant follicles among the three groups(P>0.05).There were no significant differences in abortion rate,ectopic pregnancy rate and multiple pregnancy rate among the three groups(P>0.05);the clinical pregnancy rate of the three groups was statistically significant,and that in Letrozole+uFSH-HP group was higher than the other two groups(P<0.05).Conclusion Letrozole combined with uFSH-HP has better ovulation induction effect,it may be related to promote the excretion more of mature follicle after the drug acts on the human body,the mature follicle also promotes the effect of the drug,and significantly improves the clinical pregnancy rate.
