缬沙坦联合氨氯地平治疗中老年高血压并发蛋白尿256例  

Valsartan Combined with Amlodipine in the Treatment of 256 Cases of Middle-aged and Elderly Hypertension Complicated with Proteinuria

摘　　要：目的探究缬沙坦联合氨氯地平治疗中老年高血压并发蛋白尿的效果。方法方便抽取2017年6月—2019年6月在该院接受治疗的中老年高血压并发蛋白尿患者256例为研究对象,回顾分析256例患者的临床治疗资料,所有患者均予以缬沙坦联合氨氯地平治疗,记录所有患者用药前后SBP、DBP、24 h尿蛋白定量变化情况及药物相关不良反应发生情况。结果治疗前患者SBP、DBP、24 h尿蛋白定量水平分别为(155.23±6.58)mmHg、(100.63±4.94)mmHg、(10.62±1.64)g均高于治疗后(118.69±3.94)mmHg、(84.93±2.43)mmHg、(4.62±0.64)g,差异有统计学意义(t=54.875、37.578、44.975,P<0.05);256例中老年高血压并发蛋白尿患者中,有2例患者出现面部潮红,1例患者出现踝部水肿,1例患者出现低血压症,出现不良反应情况的患者4例,不良反应发生率为1.56%。所有调查患者中,没有患者出现两种及两种以上不良反应,对于4例出现不良反应的患者,均为进行相关治疗,采取停药的方式,停药数天后患者的不良反应症状消失,未出现病情加重、死亡等情况。结论对中老年高血压并发蛋白尿患者行缬沙坦联合氨氯地平治疗能够改善患者的SBP、DBP、24 h尿蛋白定量水平,不良症状发生概率较少。Objective To explore the effect of valsartan combined with amlodipine in the treatment of middle-aged and elderly hypertension complicated with proteinuria.Methods 256 middle-aged and elderly hypertensive complicated with proteinuria patients who were treated in the hospital from June 2017 to June 2019 were selected as the research objects.The clinical treatment data of 256 patients were retrospectively analyzed.All patients were treated with valsartan combined with amlodipine,record the quantitative changes of SBP,DBP,24 h urine protein and the occurrence of drug-related adverse reactions in all patients before and after medication.Results The quantitative levels of SBP,DBP,and 24 h urine protein in patients before treatment were(155.23±6.58)mmHg,(100.63±4.94)mmHg,and(10.62±1.64)g,respectively,higher than those after treatment(118.69±3.94)mmHg and(84.93±2.43)mmHg,(4.62±0.64)g,the difference was statistically significant(t=54.875,37.578,44.975,P<0.05);out of 256 middle-aged and elderly hypertension patients with proteinuria,2 patients had facial flushing,1 patient developed ankle edema,one patient developed hypotension,the number of patients with adverse reactions was 4,and the adverse reaction rate was 1.56%.Of all the patients investigated,no patient had two or more adverse reactions,and for the four patients who had adverse reactions,all were conducted relevant treatment,taking a discontinuation of the drug,after a few days of discontinuation of the patient’s adverse symptoms disappeared,no worsening of the disease,death,etc.Conclusion Valsartan combined with amlodipine treatment for middle-aged and elderly hypertensive patients with proteinuria can amelioreate the quantitative levels of SBP,DBP,and 24 h urine protein in patients,the probability of adverse symptoms in patients is less.
