摘 要:目的:探讨钙化性腱膜纤维瘤的高频彩色多普勒超声表现,分析超声对其临床诊断价值。方法:回顾性分析12例经手术病理证实的钙化性腱膜纤维瘤高频超声表现,总结其超声声像图特征。结果:12例CAF患者,男7例,女5例,年龄3~15岁,平均8岁。位于手部6例,位于足部5例,位于上臂近腋窝处1例。二维超声多表现为形态不规则、边界多不清晰、内部回声不均匀的实性肿物(8例为实性低回声,3例为实性高回声,1例为实性强回声),其中7例内可见钙化强回声(1例为密集点状钙化,3例为散在粗大钙化,3例为散在点状钙化);2例病灶后方回声衰减;彩色多普勒显示3例病灶内可见少许血流信号(血流Adler分级1级),9例病灶内未见明显血流信号。结论:高频超声对钙化性腱膜纤维瘤病灶的大小、形态、内部回声及血流情况有一定的特异性,结合其年龄及发病部位可提高该疾病的诊断准确率。Objective:To evaluate the clinical diagnostic value of high frequency color Doppler ultrasonography in calcifying aponeurotic fibroma(CAF).Methods:Ultrasonographic findings of 12 cases of CAF confirmed by surgery and pathology were analyzed retrospectively.Results:There were 12 CAF patients(7 males and 5 females)with an average age of 8 years(3~15 years).Six cases were in the hand,5 cases were located at the bottom of the foot,and 1 case was in the upper arm near the axilla.Two-dimensional ultrasound showed a solid mass with irregular shape,unclear boundary,and uneven internal echo(8 cases were hypoechoic,3 cases were hyperechoic,and 1 case was strong echo).Calcification was found in 7 cases(1 case was dense punctated calcification,and 3 cases were scattered thick calcification,and 3 cases were scattered dot calcification),and 2 cases were found posterior echo attenuation.Color Doppler showed few blood flow signals in 3 cases(grade 1,Adler grading)and no obvious blood flow signals in 9 cases.Conclusion:High frequency ultrasound has certain specific features in size,shape,internal echo and blood flow of calcified aponeurotic fibroma,combined with the location and age can improve the acuracy of diagnosis.
