腺叶加峡部切除术联合同侧气管食管沟清扫术治疗甲状腺乳头状癌的临床观察

摘　　要：目的探讨腺叶加峡部切除术联合同侧气管食管沟清扫术治疗甲状腺乳头状癌的临床疗效。方法回顾性分析120例甲状腺乳头状癌患者的临床资料,根据手术方式的不同分为腺叶加峡部切除术联合同侧气管食管沟清扫术(观察组)70例与腺叶加峡部切除术组(对照组)50例。比较两组患者的手术疗效、术后复发率。结果观察组的手术时间长于对照组,术中出血量、术后引流量多于对照组,差异均有统计学意义(t分别=5.13、4.40、4.14,P均<0.05)。观察组的低钙血症、低甲状旁腺激素(PTH)、声嘶等总发生率与对照组比较,差异无统计学意义(χ2=1.19,P>0.05)。观察组的末次随访复发率低于对照组,差异有统计学意义(χ2=5.02,P<0.05)。结论腺叶加峡部切除术联合同侧气管食管沟清扫术虽然在一定程度上增加了手术的难度,但其能降低甲状腺乳头状癌患者术后复发的风险,是安全可行的。Objective To investigate the clinical effect of adenoidectomy and isthmectomy combined with ipsilateral tracheoesophageal ditch dissection in the treatment of papillary thyroid carcinoma.Methods The data of 120 cases of papillary thyroid carcinoma were analyzed retrospectively.The patients were divided into two groups according to the different operation methods:70 cases in the observation group and 50 cases in the control group.The operative effect and recurrence rate between two groups were compared.Results The operation time of the observation group was longer than that of the control group,and the volume of bleeding during the operation and the volume of drainage after the operation were more than those of the control group,the differences were statistically significant(t=5.13,4.40,4.14,P<0.05).The total incidence of hypocalcemia,low PTH and hoarseness between two groups was not significantly different(χ2=1.19,P>0.05).The control group(χ2=5.02,P<0.05).Conclusion Adenoidectomy and isthmectomy combined with ipsilateral tracheoesophageal ditch dissection may increase the difficulty of operation to a certain extent,but it can reduce the risk of postoperative recurrence,which is safe and feasible.
