奥洛他定滴眼液等药物联合治疗重度角膜缘型春季角结膜炎的临床观察

摘　　要：目的:探讨奥洛他定联合0.1%氟米龙滴眼液、妥布霉素地塞米松眼膏治疗重度角膜缘型春季角结膜炎(VKC)的临床疗效。方法:回顾性分析2014年1月—2019年12月汕头大学·香港中文大学联合汕头国际眼科中心确诊的65例(130只眼)重度角膜缘型VKC,其中男性56例,女性9例,年龄2~26岁。分为观察组35例(70只眼)和对照组30例(60只眼),观察组患者给予奥洛他定联合0.1%氟米龙滴眼液、妥布霉素地塞米松眼膏治疗。对照组患者给予埃美丁或色甘酸钠滴眼液治疗。两组疗程均为2周。比较两组治疗前后视力改善情况、临床疗效、复发率及并发症情况。结果:观察组治疗2周后有效率为88.6%、治愈率为71.4%,均高于对照组(有效率为43.3%、治愈率为10%),差异有统计学意义(均P<0.01)。观察组的复发率(11.5%)明显也低于对照组(66.7%),差异有统计学意义(P<0.05)。结论:奥洛他定联合0.1%氟米龙滴眼液、妥布霉素地塞米松眼膏治疗重度角膜缘型VKC的有效率及治愈率高,复发率低,无并发症。Objective:To investigate the clinical efficacy of olopatadine combined with 0.1%fluminone eye drops and tobramycin dexamethasone eye ointment in the treatment of severe limbal vernal keratoconjunctivitis(VKC).Methods:65 patients(130 eyes)with severe limbal VKC diagnosed by Joint Shantou International Eye Center of Shantou University and the Chinese University of Hong Kong from January 2014 to December 2019 were retrospectively analyzed,including 56 males and 9 females,aged 2-26 years.They were divided into observation group(35 cases,70 eyes)and control group(30 cases,60 eyes).The patients in the observation group were treated with olopatadine combined with 0.1%flumilone eye drops and tobramycin dexamethasone eye ointment.The patients in the control group were given emetine or sodium cromoglycate eye drops.The course of treatment was 2 weeks in both groups.The improvement of visual acuity,clinical efficacy,recurrence rate and complications were compared between the two groups before and after treatment.Results:After 2 weeks of treatment,the effective rate and cure rate of the observation group were 88.6%and 71.4%,respectively,which were higher than those of the control group(43.3%and 10%,respectively),and the differences were statistically significant(all P<0.01).The recurrence rate of the observation group(11.5%)was significantly lower than that of the control group(66.7%),the difference was statistically significant(P<0.05).Conclusion:The efficacy and cure rate of olopatadine combined with 0.1%flumion eye drops and tobramycin dexamethasone eye ointment in the treatment of severe limbal VKC are high,the recurrence rate is low,and there is no complication.
