摘 要:目的:探讨CT灌注成像在肝脏结节性病变鉴别诊断中的应用价值。方法:抽取2017年1月至2020年1月于南阳市中心医院就诊且经病理组织检查明确诊断的92例肝脏结节性病变患者为研究对象。所有患者均行CT灌注成像及超声检查,比较CT灌注成像与超声检查对良恶性病变组织的诊断符合率,比较良恶性病变组织的各CT灌注值。结果:CT灌注成像对恶性病变的诊断符合率(97.92%,47/48)高于超声检查(87.50%,42/48),P<0.05;CT灌注成像对良性病变的诊断符合率与超声检查比较差异未见统计学意义(P>0.05)。恶性病变组织的肝血容积、肝血流量、肝动脉灌注量、毛细血管表面通透性高于良性病变组织,平均通过时间低于良性病变组织(P<0.05)。结论:CT灌注成像在肝脏结节性病变鉴别诊断中有重要应用价值,可有效鉴别良恶性病变,提高恶性病变的检出率。Objective To investigate the application value of computed tomography(CT)perfusion imaging in the differential diagnosis of liver nodular lesions.Methods A total of 92 patients with liver nodular lesions who were confirmed by pathological examination and treated in Nanyang Central Hospital from January 2017 to January 2020 were selected as the research objects.All patients were subjected to CT perfusion imaging and ultrasound examination.The diagnostic coincidence rates of CT perfusion imaging and ultrasound in the diagnosis of benign and malignant lesions were compared.And the CT perfusion values of benign and malignant lesions were compared.Results The coincidence rate of CT perfusion imaging in the diagnosis of malignant lesions was 97.92%(47/48),significantly higher than the 87.50%(42/48)of ultrasound examination(P<0.05).And there was no significant difference in the coincidence rate in the diagnosis of benign lesions between CT perfusion imaging and ultrasound examination(P>0.05).The hepatic blood volume,hepatic blood flow,hepatic arterial perfusion and permeability surface area product values of malignant lesions were significantly higher than those of benign lesions,while the mean transit time of malignant lesions was significantly lower than that of benign lesions(P<0.05).Conclusions CT perfusion imaging has important application values in the differential diagnosis of liver nodular lesions,which can effectively identify benign and malignant lesions,and improve the detection rate of malignant lesions.
