高强度聚焦超声刀治疗子宫肌瘤的效果观察

摘　　要：目的:探讨高强度聚焦超声刀(HIFU)治疗子宫肌瘤的临床疗效。方法:抽取2016年1月至2019年1月新里程安钢总医院收治的266例患者进行前瞻性研究,按照随机数字表法分为对照组和观察组,每组133例。对照组采用腹腔镜子宫肌瘤剔除术,观察组采用HIFU,比较两组手术相关情况、术后即刻优良率、症状严重程度评分(SSS)、子宫肌瘤症状-生活质量问卷(UFS-QOL)、并发症发生情况及治疗后6个月的复发率。结果:观察组术中出血量少于对照组,手术时间、住院时间短于对照组,手术费用高于对照组(P<0.05);观察组术后即刻优良率(99.25%,132/133)与对照组(100.00%,133/133)比较,差异未见统计学意义(P>0.05);治疗后1个月,观察组SSS、UFS-QOL评分低于对照组(P<0.05);观察组并发症发生率(0.75%,1/133)低于对照组(6.02%,8/133),P<0.05;观察组治疗后6个月复发率(3.01%,4/133)与对照组(1.50%,2/133)比较差异未见统计学意义(P>0.05)。结论:HIFU治疗子宫肌瘤,能缩短手术时间,减少术中出血量,加快患者术后恢复,促进肌瘤的消退,改善患者临床症状,提高患者生活质量,安全可靠。Objective To investigate the efficacy of high-intensity focused ultrasound(HIFU)in the treatment of uterine fibroids.Methods A prospective study was performed,of which 266 patients admitted to New Journey Angang General Hospital from January 2016 to January 2019 were selected as research objects,and they were divided into control group and observation group by random number table method,with 133 cases in each group.The control group was treated by laparoscopic myomectomy,and the observation group was treated by HIFU.The surgical related conditions,immediately postoperative excellent rate,symptom severity score(SSS),uterine fibroid symptom-quality of life questionnaire(UFS-QOL),incidence of complications and recurrence rate 6 months after treatment were compared between the two groups.Results The blood loss in the observation group was lower than that in the control group,while the operation time and hospital stay in the observation group were shorter than those in the control group,however,the operation cost was higher than that in the control group(P<0.05).There was no significant difference in the immediately postoperative excellent rate(99.25%,132/133)of the observation group with that of the control group(100.00%,133/133),P>0.05.The SSS and UFS-QOL scores of the observation group at 1 month after treatment were lower than those of the control group(P<0.05).The incidence of complications in the observation group(0.75%,1/133)was lower than that in the control group(6.02%,8/133),P<0.05.There was no significant difference in the recurrence rate 6 months after treatment between the observation group(3.01%,4/133)and the control group(1.50%,2/133),P>0.05.Conclusions The treatment of uterine fibroids with HIFU can shorten the operation time,reduce the amount of intraoperative bleeding,accelerate the postoperative recovery of patients,promote the regression of fibroids,improve the clinical symptoms and quality of life of patients,and the treatment method is safe and reliable.
