复方苦参注射液联合化疗治疗中晚期非小细胞肺癌的近期疗效及对患者生存质量和免疫功能的影响

摘　　要：目的探讨复方苦参注射液联合化疗治疗中晚期非小细胞肺癌(NSCLC)的近期疗效及对患者生存质量和免疫功能的影响。方法选择河南省肿瘤医院2017年9月至2019年9月收治的中晚期NSCLC患者82例,依据随机数字表法分为观察组41例与对照组41例。对照组患者采用化疗治疗,观察组患者在化疗基础上结合复方苦参注射液治疗。两组均以28 d为一化疗周期,连续化疗4个周期后评价疗效。比较两组近期疗效、治疗前后卡氏(KPS)评分和免疫功能,及不良反应发生情况。结果观察组总有效率(63.41%,26/41)高于对照组(41.46%,17/41),P<0.05。两组化疗后KPS评分较化疗前增加(P<0.05),且观察组化疗后KPS评分高于对照组(P<0.05)。化疗后,观察组CD3^+、CD4^+和CD4^+/CD8^+较化疗前增加(P<0.05),且高于对照组(P<0.05)。观察组血小板减少、胃肠道反应、白细胞减少、肝肾异常和骨髓抑制发生率均低于对照组(P均<0.05)。结论复方苦参注射液联合化疗治疗中晚期NSCLC患者的近期疗效明显,且可改善患者生存质量并增强机体免疫功能。Objective To investigate the short-term effects of compound Kushen injection combined with chemotherapy on patients with intermediate-advanced non-small cell lung cancer(NSCLC),and its influence on the quality of life and immune function.Methods A total of 82 patients with intermediate-advanced NSCLC admitted to Henan Cancer Hospital from September 2017 to September 2019 were selected,and patients were divided into observation group(41 cases)and control group(41 cases)by random number table method.Patients in the control group were treated by chemotherapy,while patients in the observation group were treated with compound Kushen injection on the basis of chemotherapy.Both groups were treated by continuous chemotherapy for 4 cycles,with 28 days as a cycle,and then the clinical effects were evaluated.The short-term curative effect,preoperative and postoperative Karnofsky(KPS)score,immune function and adverse reactions of the two groups were compared.Results The total effective rate of the observation group(63.41%,26/41)was higher than that of the control group(41.46%,17/41),P<0.05.KPS scores of the two groups increased after chemotherapy(P<0.05),and KPS scores of the observation group were higher than those of the control group after chemotherapy(P<0.05).The cluster of differentiation(CD)3^+,CD4^+and CD4^+/CD8^+in the observation group increased after chemotherapy(P<0.05),and they were higher than those in the control group(P<0.05).The incidences of thrombocytopenia,gastrointestinal reaction,leucopenia,hepatorenal abnormality and myelosuppression in the observation group were lower than those in the control group(all P<0.05).Conclusions Compound Kushen injection combined with chemotherapy can improve the quality of life and immune function of patients with intermediate-advanced NSCLC.
