摘 要:目的探讨细胞角质蛋白19(CK19)、甲状腺过氧化物酶(TPO)、半乳糖血凝素-3(Gal-3)在甲状腺良恶性组织中的表达差异及其对甲状腺癌的诊断价值。方法回顾性分析梅州市人民医院2019年1月至2020年1月收治的甲状腺癌患者63例与良性甲状腺结节患者26例的临床病理资料,所有患者均经手术获取病灶组织,且均进行免疫组化检测。分析甲状腺良恶性组织CK19、TPO与Gal-3的表达情况及其对甲状腺癌的诊断价值。结果恶性组CK19与Gal-3表达阳性率明显高于良性组,恶性组TPO表达阳性率明显低于良性组,差异均有统计学意义(P<0.05);甲状腺恶性组CK19(+)Gal-3(+)TPO(-)组合表达率明显高于良性组,且良性组CK19(-)Gal-3(+)TPO(+)与CK19(-)Gal-3(-)TPO(+)表达率明显高于恶性组,差异均有统计学意义(P<0.05);甲状腺癌TPO表达情况与年龄、性别、T分期无关(P>0.05),与N分期、临床分期有关(P<0.05);多因素logistic回归分析结果显示,TPO(-)与CK19(+)是甲状腺癌的高危因素(P<0.05),得到模型Y=4.293(CK19)-3.055(TPO)-1.380;ROC曲线分析结果显示,联合诊断模型对于甲状腺癌的诊断价值(AUC=0.949)高于CK19(0.838)、Gal-3(0.696)与TPO(0.874)。结论CK19、TPO与Gal-3在甲状腺良恶性组织中的表达均有显著差异,CK19(+)Gal-3(+)TPO(-)在甲状腺癌中有较高的表达率,其中CK19联合TPO对于甲状腺癌的诊断价值最高。Objective To investigate the expression difference of CK19,TPO and Gal-3 in thyroid benign and malignant tissues and their diagnostic value for thyroid cancer.Methods The clinicopathological data of 63 cases of thyroid cancer and 26 cases of benign thyroid nodule in Meizhou People’s Hospital from January 2019 to January 2020 were analyzed retrospectively.All the patients were obtained the focus tissue by operation,and were detected by immunohistochemistry.To analyze the expression of CK19,TPO and Gal-3 in thyroid benign and malignant tissues and their diagnostic value for thyroid cancer.Results The positive rates of CK19 and Gal-3 expression in malignant group were significantly higher than those in benign group.The positive rate of TPO expression in malignant group was significantly lower than that in benign group(P<0.05).The expression rate of CK19(+)Gal-3(+)TPO(-)in thyroid malignant group was significantly higher than that in benign group,and the expression rates of CK19(-)Gal-3(+)TPO(+)and CK19(-)Gal-3(-)TPO(+)in benign group were significantly higher than those in malignant group,the differences were significant(P<0.05).TPO expression in thyroid cancer was not related to age,gender and T stage(P>0.05).It was correlated with N stage and clinical stage(P<0.05).Multivariate logistic regression analysis showed that TPO(-)and CK19(+)were the high risk factors of thyroid cancer(P<0.05).The model y=4.293(CK19)-3.055(TPO)-1.380 was obtained;ROC curve analysis showed that the diagnostic value of combined diagnosis model for thyroid cancer(AUC=0.949)was higher than CK19(0.838),Gal-3(0.696)and TPO(0.874).Conclusions The expression of CK19,TPO and Gal-3 in thyroid benign and malignant tissues were significantly different.CK19(+)Gal-3(+)TPO(-)have high expression rates in thyroid cancer,and CK19 combined with TPO has the highest diagnostic value for thyroid cancer.
