摘 要:目的观察玻璃体腔内注射康柏西普注射液(Conbercept)后,不同时间激光光凝治疗视网膜分支静脉阻塞(Branch Retinal Vein Occlusion,BRVO)的临床疗效和安全性。方法回顾性分析2016年2月至2018年6月,缺血型BRVO继发黄斑水肿患者66例66只眼的临床资料,均为单眼。先行玻璃体腔注射0.5 mg康柏西普注射液后再行激光光凝治疗,根据玻璃体腔注射后不同时间行激光光凝分为三组,分别为注射后3 d(A组);注射后7 d(B组)和注射后14 d(C组),每组患者22例22只眼。静脉阻塞区激光参数:波长532 nm,治疗功率150~350 mW,光斑直径200μm。治疗后1,3和6个月,三组患者最佳矫正视力(best corrected visual acuity,BCVA)、光学相干断层扫描(optical coherence tomography,OCT)检测黄斑中心凹厚度(central macular thickness,CMT)、眼前节以及眼压的改变。并记录不良反应发生情况。结果66例患者随访6~8个月,平均(6.77±1.4)个月。治疗后眼前节、眼压与治疗前比较无明显改变;三组治疗后视力较治疗前均有明显改善(P<0.05),其中B组治疗后6个月,BCVA优于A组和C组(P<0.05);B组治疗后1,3和6个月平均CMT优于A组和C组,差异具有统计学意义(P<0.05)。结论玻璃体腔注射康柏西普后7 d行激光光凝治疗效果最佳,对于经济条件有限的患者可作为最佳选择。Objective To observe the clinical efficacy and safety to treat branch retinal vein occlusion(BRVO)with laser photocoagulation at different time points after intravitreal injection of Conbercept.Methods A retrospective analysis was performed on the clinical data of 66 eyes of 66 patients with ischemic BRVO secondary macular edema admitted from Feb.2016 to Jun.2018,All the patients were given laser photocoagulation after the intravitreal injection of 0.5 mg Conbept.They were divided into three groups by the time point selected after the intravitreal injection,namely,3 d(Group A),7 d(Group B)and 14 d(Group C)after the injection.There were 22 patients in each group(22 eye in total).The best corrected visual acuity(BCVA)and central macular thickness(CMT)were observed 1,3 and 6 months respect after the treatment and adverse reactions were recorded.Results The BCVA was improved more remarkably in Group B than in other groups(P<0.05).In addition,the CMT in Group B was also better than in other groups,and the differences were statistically significant(P<0.05).Conclusions The best result will be achieved when laser photocoagulation is performed on the 7 th day after the intravitreal injection of Conbercept.
