术前新辅助化疗联合外科手术治疗非小细胞肺癌的临床效果

摘　　要：目的探究术前新辅助化疗联合外科手术治疗非小细胞肺癌的临床效果。方法将2013年1月至2015年5月收治的100例非小细胞肺癌患者根据治疗方法不同分为对照组和试验组,各50例。对照组进行外科手术治疗,试验组则在外科手术治疗前联合新辅助化疗(40例接受Gem+DDP化疗方案,10例接受CAP化疗方案)。比较两组的治疗效果。结果试验组的治疗总有效率高于对照组(P<0.05)。治疗1周后,两组的CD3+、CD4+、全血NK细胞比例均较治疗前降低(P<0.05),CD8+、CD4+/CD8+与治疗前比较,差异不显著(P>0.05);试验组治疗1周后的全血NK细胞比例低于对照组(P<0.05)。试验组术前肿瘤直径小于入院时(P<0.05)。两组的并发症总发生率比较,差异不显著(P>0.05);试验组的5年生存率高于对照组(P<0.05)。结论术前新辅助化疗联合外科手术治疗非小细胞肺癌的效果显著,能提高患者的5年生存率,且安全性较高。Objective To explore the clinical effect of preoperative neoadjuvant chemotherapy combined with surgical operation in the treatment of non-small cell lung cancer.Methods A total of 100 patients with non-small cell lung cancer admitted from January 2013 to May 2015 were divided into control group and experimental group according to different treatment methods,with 50 cases in each group.The control group received surgical operation treatment,while the experimental group received neoadjuvant chemotherapy before surgical operation(40 cases received Gem+DDP chemotherapy regimens,10 cases received CAP chemotherapy regimens).The therapeutic effects of the two groups were compared.Results The total effective rate of treatment of the experimental group was higher than that of the control group(P<0.05).After one week of treatment,CD3+,CD4+and NK cells ratio in whole blood in the two groups were lower than those before treatment(P<0.05),but there were no significant differences in CD8+,CD4+/CD8+between before and after treatment(P>0.05);NK cells ratio in whole blood in the experimental group was lower than that in the control group after 1 week of treatment(P<0.05).The tumor diameter in the experimental group before operation was smaller than that at admission(P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05);the 5-year survival rate of the experimental group was higher than that of the control group(P<0.05).Conclusion Preoperative neoadjuvant chemotherapy combined with surgical operation in the treatment of non-small cell lung cancer has significant effect,it can improve the 5-year survival rate of patients,and has higher safety.
