伏立康唑单独或联合硝苯地平对茄病镰刀菌复合体及白念珠菌的体外抗菌活性研究

摘　　要：目的测定伏立康唑对临床分离的茄病镰刀菌复合体和白念珠菌的抗菌效果,并分析其与钙通道阻滞剂硝苯地平联合使用的抗菌效果。方法参照美国临床实验室标准化委员会的M27-Ed4及M38-A3方案,采用微量液体稀释法测定单独使用伏立康唑、硝苯地平对30株茄病镰刀菌复合体和15株白念珠菌的最低抑菌浓度(MIC),采用棋盘法测定药物联合使用的抗真菌效果。结果伏立康唑对茄病镰刀菌复合体和白念珠菌的MIC范围分别是0.25~8.00μg/ml与2.00~64.00μg/ml。伏立康唑联合应用硝苯地平时,对所有的茄病镰刀菌复合体(30/30)无协同效应,无拮抗效应;对86.7%(13/15)的白念珠菌有协同效应,对所有菌株无拮抗效应,药物敏感性增加(P<0.001)。结论伏立康唑联合应用硝苯地平可增加白念珠菌对伏立康唑的药物敏感性,有协同效应,对茄病镰刀菌复合体无协同效应,无拮抗效应。Objective To evaluate in vitro antifungal effect of voriconazole or voriconazole combined with nifedipine against clinically isolated F.solani.and C.ablibans.Methods According to the M27-Ed4 and M38-A3 schemes proposed by Clinical and Laboratory Standards Institute(CLSI),the minimum inhibitory concentration(MIC)of voriconazole and nifedipine on 30 strains of F.solani.and 15 strains of C.ablibans.was measured by the microdilution method.The antifungal effect of the combination of two drugs was assessed by the checkerboard method.Results The MIC ranges of voriconazole against F.solani.(30 strains)and C.ablibans.(15 strains)were 0.25-8.00μg/ml and 2.00-64.00μg/ml,respectively.When applied in combination with nifedipine in vitro,there was no synergistic effect or antagonistic effect on all F.solani.strains(30/30).There was synergistic effect on 86.7%(13/15)of C.ablibans.strains,whereas no antagonistic effect on all C.ablibans.strains.The drug sensitivity was significantly enhanced(P<0.01).Conclusion Nifedipine can increase the drug sensitivity of C.ablibans.to voriconazole,which has a synergistic effect.It exerts no synergistic or antagonistic effect on F.solani.
