金钱草联合中成药治疗胆囊结石临床研究

摘　　要：目的:观察金钱草联合中成药治疗胆囊结石的临床疗效。方法:将确诊为胆囊结石的90例患者按随机数字表法分为结合治疗组、对照组各45例。2组均予以熊去氧胆酸胶囊治疗,结合治疗组在此基础上加予胆石利通片、消炎利胆片、金钱草水煎剂治疗,共治疗4周。记录2组治疗前后胆囊收缩素(CCK)、血管活性肠肽(VIP)水平及结石个数、结石直径,比较2组临床疗效。结果:与治疗前比较,2组血清CCK水平升高,VIP水平降低,胆囊结石个数减少、结石直径缩小(P<0.05)。治疗后,结合治疗组较对照组CCK水平升高更显著,VIP水平降低更显著,结石个数减少、结石直径缩小更显著(P<0.05)。对照组总有效率为86.67%,结合治疗组为97.78%,2组比较,差异有统计学意义(P<0.05)。结论:金钱草联合中成药治疗胆囊结石效果显著,能够有效减轻患者的临床症状,减少其结石个数及缩小结石直径,可能与降低VIP水平、提高CCK水平有关。Objective:To observe the curative effect of the therapy of Herba Lysimachiae combined with Chinese patent medicine on patients with cholecystolithiasis. Methods:A total of 90 patients with cholecystolithiasis were randomly divided into the combined treatment group and the control group, 45 cases in each group. The control group was treated with ursodeoxycholic acid capsules;the combined treatment group was additionally treated with Danshi Litong tablets,Xiaoyan Lidan tablets and decocted Herba Lysimachiae based on the treatment of the control group. Both groups were treated for four weeks. Changes in the levels of cholecystokinin(CCK) and vasoactive intestinal peptide(VIP), the number and diameter of gallstones and clinical effect were recorded before and after treatment in the two groups. Results: Compared with those before treatment, the CCK levels in serum were increased in both groups;the VIP levels, the number and diameter of gallstones were decreased in both groups(P<0.05). After treatment,the CCK level in the combined treatment group was increased more significantly than that in the control group,and the VIP level,the number and diameter of gallstones were decreased more significantly than those in the control group(P<0.05). The total effective rate was 86.67% in the control group and 97.78% in the combined treatment group,the difference being significant(P<0.05). Conclusion:The therapy of Herba Lysimachiae combined with Chinese patent medicine has a significant effect in treating patients with cholecystolithiasis,which can effectively reduce the clinical symptoms,and relatively reduce the number and diameter of gallstones,whose mechanism may be related to the reduction of VIP level and increase of CCK level.
