腹腔镜手术治疗老年晚期大肠癌合并转移患者的临床效果

摘　　要：目的观察腹腔镜手术治疗老年晚期大肠癌合并转移患者的临床效果。方法选取2017年12月至2018年12月沈阳市肛肠医院收治的98例老年晚期大肠癌合并转移患者,按照患者接受手术治疗方式的不同分为腹腔镜组和开腹组,每组49例。腹腔镜组给予腹腔镜手术治疗,开腹组给予传统开腹手术治疗,比较两组患者的手术相关指标、手术切除标本病理学指标及并发症发生情况。结果腹腔镜组术中出血量少于开腹组,术后下床时间、住院时间短于开腹组,差异均有统计学意义(均P<0.05);两组在手术时间、术后排气时间、肿瘤大小、结肠标本长度、直肠标本长度及淋巴结清扫个数方面比较,差异均无统计学意义(均P>0.05);腹腔镜组并发症发生率低于开腹组,差异有统计学意义(P<0.05)。结论在为老年晚期大肠癌合并转移患者实施治疗的过程中,腹腔镜手术具有微创、并发症少、术后恢复快等特点,相较于传统开腹手术具有更加安全、有效的治疗效果。Objective To observe the clinical effect of laparoscopic surgery in the treatment of elderly patients with advanced colorectal cancer complicated with metastasis.Methods From December 2017 to December 2018,98 elderly patients with advanced colorectal cancer complicated with metastasis were enrolled.They were divided into laparoscopy group and laparotomy group according to different surgical treatment methods,with 49 cases in each group.The patients in laparoscopy group treated with laparoscopic surgery were taken as laparoscopic group,the patients in laparotomy group treated with traditional laparotomy were taken as traditional group,and the surgical effects of the two groups were compared and analyzed.Results The intraoperative blood loss in the laparoscopy group was less than that of the open group,and the postoperative time out of bed and hospitalization time were shorter than those in the laparotomy group,the differences were statistically significant(all P<0.05).There was no significant difference in operation time,tumor size,length of colon specimen,length of rectal specimen,and the number of lymph node dissections(all P>0.05).The incidence of complications in the laparoscopy group was lower than that in the laparotomy group,and the difference was statistically significant Significance(P<0.05).Conclusion In the process of treatment for elderly patients with advanced colorectal cancer complicated with metastasis,laparoscopic surgery has the remarkable characteristics of minimally invasive,less complications,rapid postoperative recovery,etc.While making up for the short comings of traditional laparotomy,it has the curative effect,is safer and more effective.
