聚维酮碘消毒中粘弹剂角膜上皮保护在康柏西普球内注药中的应用研究

摘　　要：目的:探讨聚维酮碘消毒过程中粘弹剂角膜上皮保护在抗VEGF治疗过程中的应用价值。方法:前瞻性研究,将300例住院进行康柏西普球内注药的患者随机分为两组,对照组术前利用5%国产聚维酮碘进行消毒;观察组在消毒过程中,首先利用粘弹剂覆盖角膜,进行角膜上皮保护,再利用5%国产聚维酮碘进行消毒。对患者术后结膜囊带菌率及疼痛等不良反应发生率进行评估。结果:两组手术前后视力总体比较,差异有统计学意义(Z=-2.968,P=0.002);两组患者消毒前带菌率比较,差异无统计学意义(χ~2=0.138,P=0.371);观察组消毒后带菌率为3.33%,对照组为2.67%,差异无统计学意义(χ~2=0.138,P=0.371),两组消毒前后总体带菌率之间进行比较,差异有统计学意义(χ~2=6.635,P=0.003)。观察组消毒后不良反应发生率78.67%,高于对照组的24.00%,差异有统计学意义(χ~2=154.000,P=0.000)。两组消毒前后VAS分值分别为(8.51±0.32)分及(4.46±0.15)分,两者比较差异有统计学意义(P=0.000)。结论:粘弹剂角膜上皮保护能够明显减轻5%聚维酮碘消毒过程中患者的术后反应,提高患者的舒适感,该方法简便易行,易于推广及应用。Objective:To explore the application value of viscoelastic corneal epithelial protection in the injection of Conbercept in ball during 5% domestic Povidone Iodine disinfection.Method:Prospective study,300 inpatients were randomly divided into two groups.5% domestic Povidone Iodine was used for disinfection before operationin the control group.Viscoelastic agent was used to cover the cornea for epithelial protection,and then 5% domestic Povidone Iodine was used for disinfectionin the experimental group.The infection rate of conjunctival sac bacteria and pain were evaluated.Result:Compared the visual acuity of the two groups before and after operation,the difference was statistically significant (Z=-2.968,P=0.002).Compared the bacterial bearing rate between the two groups before sterilization,there was no significant difference (χ~2=0.138,P=0.371).Compared the bacterial bearing rate between the two groups after sterilization,there was no significant difference (χ~2=0.138,P=0.371).The bacterial bearing rate before sterilization of two groups were different from that after sterilization (χ~2=6.635,P=0.003).The incidence of adverse reactions in the control group (78.67%) was higher than 24.00% in the observation group,the difference was statistically significant (χ~2=154.000,P=0.000).The VAS scores of the two groups after disinfection were (8.51±0.32) points and (4.46±0.15) points respectively,the difference between the two groups was statistically significant (P=0.000).Conclusion:Viscoelastic corneal epithelial protection can significantly reduce the postoperative response of patients in the process of 5% Povidone Iodine disinfection,and improve the comfort of patients.This method is simple,easy to promote and apply.
