血清脂联素与子宫内膜癌的相关性

摘　　要：目的探讨血清脂联素(adiponectin,APN)与子宫内膜癌的相关性。方法选择78例子宫内膜癌患者作为观察组,同期选择78例不典型子宫内膜增生患者作为对照组。检测2组血清APN含量,并分析其与患者的临床资料的相关性。结果2组的年龄、体重指数、产次、孕次等对比差异无统计学意义(P>0.05)。观察组的血清APN水平显著低于对照组(P<0.05),而FBG、FINS、HOMA-IR水平都显著高于对照组(P<0.05)。在观察组78例患者中,随着分化程度降低、临床分期增加与淋巴结转移,血清脂联素水平显著降低(P<0.05)。Logistic回归模型分析显示APN、FINS、HOMA-IR为影响子宫内膜癌发病的主要因素(P<0.05)。结论血清APN在子宫内膜癌中的表达显著下降,与肿瘤的病理特征相关,也是子宫内膜癌发生的重要因素。Objective To investigate the correlation between serum adiponectin(APN)and endometrial carcinoma.Methods 78 patients with endometrial carcinoma diagnosed were selected as the observation group.78 patients with atypical endometrial hyperplasia were selected as the control group.The serum APN levels of the 2 groups were detected,and the clinical data of the patients were investigated and the influencing factors were analyzed.Results There were no significant differences in age,body mass index,parity,and pregnancy compared between the 2 groups(P>0.05).The serum APN level of the observation group were significantly lower than that of the control group(P<0.05),while the levels of FBG,FINS and HOMA-IR were significantly higher than the control group(P<0.05).In 78 patients in the observation group,serum adiponectin levels were significantly decreased with decreasing degree of differentiation,clinical stage and lymph node metastasis(P<0.05).Logistic regression model analysis showed that APN,FINS and HOMA-IR were the main factors affecting the pathogenesis of endometrial carcinoma(P<0.05).Conclusion The expression of serum APN in endometrial carcinoma is significantly decreased,which is related to the pathological features of the tumor,and is also an important influencing factor leading to the occurrence of endometrial carcinoma.
