牙周病治疗前后的上颌牙槽骨骨密度的CBCT测量分析

摘　　要：目的:比较经过牙周基础治疗前后的上颌牙槽骨密度的变化情况,牙周炎症控制后,是否骨密度的变化可以作为判断牙周炎症控制的客观指标。方法:随机选取10名慢性牙周病患者,测量上颌后牙牙周基础治疗前、后牙周袋的深度。同时CBCT(锥体束CT)分别测量其上颌牙槽骨5个位点的皮质骨和松质骨的HU(Hounsfield Unit,灰度值)。每组数据量为10×5=50个。使用SPSS 22.0统计软件进行分析,采用配对t检验,检验水平为α<0.05。结果:治疗前、治疗后牙周袋的深度为(5.16±0.96)、(3.60±0.66)mm;两组间差异有统计学意义(P=0.003)。治疗前、治疗后皮质骨的HU值分别为979.48±114.80和1074.48±119.57,差异无统计学意义(P=0.095)。治疗前、治疗后松质骨的HU值分别为504.22±74.38和440.84±93.21,差异有统计学意义(P=0.037)。结论:牙周病经牙周基础治疗后,炎症消退,牙周袋变浅,松质骨密度减少。Objective:To compare the depth of periodontal pocket and density of maxillary alveolar bone before and after periodontal treatment.After the control of periodontal inflammation,whether the change of bone mineral density can be an objective indicator for the control of periodontal inflammation.Method:Ten patients with chronic periodontal disease were randomly selected to be measured the depth of the periodontal pocket before and after the initial periodontal treatment of the maxillary posterior teeth.At the same time,the HU(Hounsfield Unit)of the cortical bone and the cancellous bone at 5 sites of the maxillary alveolar bone were record by Cone beam CT(CBCT).The amount of data per group was 10×5=50.The SPSS 22.0 statistical software and the paired t test was used for analysis,and the test level wasα<0.05.Result:The depth of periodontal pockets before and after treatment was(5.16±0.96)mm and(3.60±0.66)mm,respectively.There was statistical significance between the two groups(P=0.003).The HU values of before treatment and after treatment cortical bone were 979.48±114.80 and 1074.48±119.57,respectively,and no statistically significant difference was shown(P=0.095).The HU values of before treatment and after treatment cancellous bone were 504.22±74.38 and 440.84±93.21,respectively,and the statistically significant difference was calculated(P=0.037).Conclusion:After periodontal treatment of periodontal disease,inflammatory reaction disappeared and periodontal pocket became shallower,cancellous bone density decreased as well.
