经肛肠减压后3D腹腔镜根治术治疗梗阻性结肠癌患者的临床效果

摘　　要：目的:分析经肛肠减压后3D腹腔镜根治术治疗梗阻性结肠癌患者的临床效果。方法:选取本院梗阻性结肠癌患者69例行回顾性研究,其中行传统开腹结肠癌根治手术的34例为传统组,行经肛肠减压后3D腹腔镜根治术的35例为腹腔镜组。对比两组手术时间、术中失血量、淋巴结清扫量、排气时间、尿管留置时间、住院时间及并发症状况。结果:腹腔镜组手术时间短于传统组,术中失血量少于传统组(P<0.05);两组淋巴结清扫数量差异无统计学意义(P>0.05)。腹腔镜组住院时间、尿管留置时间短于传统组,排气时间早于传统组(P<0.05);腹腔镜组并发症发生率为5.71%(2/35),传统组为11.76%(4/34),差异无统计学意义(P>0.05)。结论:与传统开腹结肠癌根治手术相比,经肛肠减压后3D腹腔镜根治术治疗梗阻性结肠癌,可改善手术效果,促进康复,且并发症少。Objective:To analyze the clinical effect of 3D laparoscopic radical resection after anorectal decompression in treatment of obstructive colon cancer.Method:Sixty-nine patients with obstructive colon cancer in our hospital were selected for a retrospective study,among which 34 patients in the traditional group underwent traditional open radical resection of colon cancer and 35 patients in the laparoscopic group underwent 3D laparoscopic radical resection after anorectal decompression.The operative time,intraoperative blood loss,lymph node dissection,exhaust time,urinary catheter indwelling time,hospital stay and complications were compared between the two groups.Result:The operation time of the laparoscopic group was shorter than that of the traditional group,and the intraoperative blood loss was less than that of the traditional group(P<0.05).The amount of lymph node dissection in two groups had no significant difference(P>0.05).The length of stay and indwelling time in the laparoscopic group were shorter than those in the traditional group,the exhaust time in the laparoscopic group was earlier than that in the traditional group(P<0.05).The incidence of complications was 5.71%(2/35)in the laparoscopic group and 11.76%(4/34)in the traditional group,with no significant difference(P>0.05).Conclusion:Compared with traditional open radical resection of colon cancer,3D laparoscopic radical resection of obstructive colon cancer after anorectal decompression can improve the surgical effect and promote rehabilitation,with fewer complications.
