梅克尔憩室伴消化道出血1例并文献复习

摘　　要：梅克尔憩室(MD)是先天性肠道畸形,在临床上比较常见,其并发症主要有消化道出血、肠梗阻、消化道穿孔、憩室炎等。消化道出血是内科的常见疾病,包括上消化道出血(消化性溃疡伴出血、食管胃底静脉曲张破裂出血、恶性肿瘤伴出血、急性胃黏膜病变等)、下消化道出血(血管畸形、小肠出血、结肠恶性肿瘤伴出血、MD伴出血等)。对于临床上原因不明的消化道出血,应该扩大诊断的思路,考虑少见病——MD的可能性。本文收集了1例成人MD以消化道出血为主要表现的病例,并通过对临床资料的分析和相关文献的回顾,探讨了该疾病的临床特点和诊疗要点。Meckel′s diverticulum(MD)is a congenital intestinal malformation,which is common in clinic.The main complications are gastrointestinal hemorrhage,intestinal obstruction,digestive tract perforation,diverticulitis,etc.Gastrointestinal hemorrhage is a common disease in internal medicine,including upper gastrointestinal hemorrhage(peptic ulcer with hemorrhage,esophageal and gastric varices rupture hemorrhage,malignant tumor with hemorrhage,acute gastric mucosal lesions,etc.)and lower gastrointestinal hemorrhage(vascular malformation,small intestinal bleeding,colonic malignant tumor with hemorrhage,MD with hemorrhage,etc.).For clinically unexplained gastrointestinal bleeding,the idea of diagnosis should be expanded to consider the possibility of rare disease-MD.This paper collected a case of adult with MD mainly manifested as the gastrointestinal hemorrhage,and through the analysis of clinical data and review of relevant literature,discussed the clinical characteristics and treatment points of this disease.
