摘　　要：本研究针对医疗资源在不同地区存在差异的现状,在长三角一体化的大背景下,探索建立基于国家和区域卫生信息化的上海市和江苏省南通市两地医疗信息和医疗资源的共享模式和支撑体系,在确保数据安全和个人隐私的前提下,实现了医疗数据的连续性和在两地的共享。针对重点患者,通过远程会诊系统和转诊系统,提供个性化的诊疗服务,既提高了优质资源服务的覆盖范围,又为南通市的患者提供了适宜的医疗服务,对跨地区的医疗数据共享和协同服务进行了探索。This paper explores the establishment of the sharing mode and supporting system of medical information and medical resources in Shanghai and Nantong,and realizes the continuity and sharing of medical data in both places under the premise of ensuring data security and personal privacy.The practice of interconnection between Shanghai and Nantong has a good demonstration function and show the possibilities of interprovincial medical data sharing and collaboration.
