摘 要:目的分析微生态肠内营养支持联合重组人生长激素对重症胰腺炎患者的预后效果。方法选取2017年7月至2019年7月收治的87例重症胰腺炎患者住院资料,采用随机分组方式将其分为研究组(n=44)和对照组(n=43),所有患者均给予重组人生长激素药物治疗,对照组在药物治疗基础上联合实施常规肠内营养支持,研究组在药物治疗基础上联合实施微生态肠内营养支持,对比两组患者接受2周不同肠内营养支持后各体征缓解时间和肠内菌群数量改善效果。结果研究组腹胀、腹痛及呕吐缓解时间分别为(2.34±0.29)d、(3.41±0.52)d、(2.95±0.49)d,均显著低于对照组,两组比较差异均有统计学意义(均P<0.05);研究组肠内菌群肠球菌、肠杆菌、双歧杆菌及乳杆菌总数分别为(5.26±1.03)In/d、(7.16±1.89)In/d、(5.97±1.68)In/d、(7.22±1.92)In/d,肠球菌及肠肝菌均低于对照组,双歧杆菌及乳杆菌均显著高于对照组,两组比较差异均有统计学意义(均P<0.05)。结论重症胰腺炎患者在重组人生长激素治疗基础上联合微生态肠内营养支持,可有效调节肠道菌群微生态环境,抑制肠道菌群移位和保护肠道屏障功能,改善全身炎症反应,增强近期预后效果。Objective To analyze the effect of microecological enteral nutrition support combined with recombinant human growth hormone on the prognosis of patients with severe pancreatitis.Methods 87 hospitalized patients with severe pancreatitis from July 2017 to July 2019 were randomly divided into two groups:study group(n=44)and control group(n=43).All patients were treated with recombinant human growth hormone,and the control group was treated with conventional enteral nutrition support on the basis of drug treatment,and the study group was treated with microecological enteral nutrition support on the basis of drug treatment.After two weeks of different enteral nutrition support,the abdominal physical signs relief time and the improvement effect of number of intestinal flora in the two groups were compared.Results The relief time of abdominal distention,abdominal pain,and emesis in the study group were(2.34±0.29)d,(3.41±0.52)d,(2.95±0.49)d,which were significantly lower than those in the control group(all P<0.05).The total number of enterococcus,enterobacterium,bifidobacterium,and lactobacillus in the study group were(5.26±1.03)In/d,(7.16±1.89)In/d,(5.97±1.68)In/d,(7.22±1.92)In/d,the total number of enterococcus and enterobacterium were significantly lower than those in the control group,the total number of bifidobacterium and lactobacillus were significantly higher than those in the control group,with statistically significant differences between the two groups(all P<0.05).Conclusion On the basis of recombinant human growth hormone treatment,microecological enteral nutrition support for severe pancreatitis patients can effectively regulate the microecological environment of intestinal flora,inhibit the displacement of intestinal flora,protect the intestinal barrier function,improve systemic inflammatory response,and enhance the short-term prognosis.
