经口间歇双腔球囊导管管饲法在脑卒中后吞咽障碍患者中的应用

摘　　要：目的探讨经口间歇双腔球囊导管管饲法在脑卒中后吞咽障碍患者中的应用效果。方法将脑卒中后吞咽障碍患者60例分为观察组和对照组,各30例。两组均给予常规吞咽康复训练,观察组采用经口间歇双腔球囊导管管饲法,对照组采用留置鼻胃管管饲法,观察两组住院14 d洼田饮水试验评级情况、日常生活活动能力评分以及吸入性肺炎发生情况。结果治疗后,两组洼田饮水试验评级较入院时均有改善,观察组总有效率显著高于对照组(83.33%比43.33%);观察组日常生活活动能力评分高于治疗前(P<0.05),且显著高于对照组(P<0.05);观察组吸入性肺炎发生率明显低于对照组(6.67%比26.67%)。结论经口间歇双腔球囊导管管饲法能有效促进脑卒中后吞咽障碍患者吞咽功能恢复,提高患者日常生活活动能力,降低吸入性肺炎发生率,值得临床推广应用。Objective To investigate the effect of intermittent double-lumen balloon catheter feeding through mouth in patients with dysphagia after stroke.Methods 60 patients with dysphagia after stroke were divided into an observation group and a control group,with 30 cases in each group.Both groups took routine swallowing rehabilitation training.The observation group were fed with intermittent double-lumen balloon catheter through their mouths,and the control group with indwelling nasogastric tube.The ratings of the swallow water test,the activities of daily living(ADL),and the incidences of aspiration pneumonia in the two groups were observed for 14 days at hospital.Results After the treatment,the ratings of the swallow water test of the two groups were improved.The total effective rate was significantly higher in the observation group than in the control group(83.33%vs.43.33%).The score of ADL in the observation group was higher after than before the treatment(P<0.05),and was significantly higher than that in the control group(P<0.05).The incidence of aspiration pneumonia in the observation groupwas significantly lower than that in the control group(6.67%vs.26.67%).Conclusion Intermittent double-lumen balloon catheter feeding through mouth can effectively promote the recovery of swallowing function in patients with dysphagia after stroke,improve their ADL,and reduce the occurrence of aspiration pneumonia,so it is worthy of clinical application.
