经阴道彩超与腹部彩超联合血清肿瘤标志物对卵巢癌的诊断价值

摘　　要：目的探讨经阴道彩超与腹部彩超联合血清肿瘤标志物氧化酶-1(COX-1)、糖类抗原125(CA125)对卵巢癌的诊断价值。方法选取本院于2017年4月至2019年4月收治的194例卵巢癌患者,其中,95例卵巢恶性肿瘤(A组),99例卵巢良性肿瘤(B组),均经病理学检查确诊,有完整的经阴道彩超与腹部彩超及血清肿瘤标志物的检查资料,回归统计各项检查资料,并对病理结果进行对照,就各项检查的优缺点进行分析。结果 A、B两组CA125、COX-1浓度比较,差异显著(P <0.05)。超声检查恶性肿瘤多具有不规则的形态,且多为实质性,而良性肿块有规则形态,边界比较整齐且清晰;良、恶性肿块超声评分比较,差异显著(P <0.05)。经腹部超声检查者,其血流信号显示为丰富血流。CA125诊断敏感度为74.74%(71/95)、特异度91.92%(91/99)、准确性83.51%(162/194);COX-1的敏感度为86.32%(82/95)、特异度93.94%(93/99)、准确性90.21%(175/194);经腹部超声的敏感度为82.11%(78/95)、特异度90.91%(90/99)、准确性86.60%(168/194);经阴道超声的敏感度为86.32%(82/95)、特异度93.94%(93/99)、准确性90.21%(175/194);联合检查的敏感度为96.84%(92/95)、特异度88.89%(88/99)、准确性92.78%(180/194)。结论在卵巢癌诊断中应用彩超联合血清肿瘤标志物,能提高敏感度,减少漏诊,临床应用价值突出。Objective To investigate the diagnostic value of transvaginal color Doppler ultrasound and abdominal color Doppler ultrasound combined with serum tumor marker cyclooxygenase-1(COX-1)and carbohydrate antigen 125(CA125)in ovarian cancer.Methods A total of 194 patients with ovarian cancer admitted to our hospital from April 2017 to April 2019 were selected.Among them,95 cases of ovarian malignant tumors(group A)and 99 cases of ovarian benign tumors(group B)were diagnosed by pathological examinations,there are complete transvaginal color Doppler ultrasound,abdominal color Doppler ultrasound and serum tumor markers examination data,regression statistics of various examination data,and comparison of pathological results,analysis of the advantages and disadvantages of each examination.Results The CA125 and COX-1 concentrations of A and B groups were signifi cantly diff erent(P<0.05).Ultrasound examination of malignant tumors has irregular shapes and most of them are substantial,while benign tumors have regular shapes,with relatively neat and clear borders;the ultrasound scores of benign and malignant tumors have signifi cant diff erences(P<0.05).The blood fl ow signal of abdominal ultrasound examination showed abundant blood fl ow.The diagnostic sensitivity of CA125 is 74.74%(71/95),the specifi city is 91.92%(91/99),and the accuracy is 83.51%(162/194);the sensitivity of COX-1 is 86.32%(82/95)and the specifi city 93.94%(93/99),accuracy 90.21%(175/194);sensitivity of transabdominal ultrasound is 82.11%(78/95),specifi city 90.91%(90/99),accuracy 86.60%(168/194);the sensitivity of transvaginal ultrasound is 86.32%(82/95),the specifi city is 93.94%(93/99),and the accuracy is 90.21%(175/194);the sensitivity of the combined examination is 96.84%(92/95),specifi city 88.89%(88/99),accuracy 92.78%(180/194).Conclusion The application of color Doppler ultrasound combined with serum tumor markers in the diagnosis of ovarian cancer can improve sensitivity,reduce missed diagnosis,and have outstanding clinical application value.
