结直肠癌癌组织NPRL2、PIVKA-Ⅱ的表达水平及临床意义

摘　　要：目的研究结直肠癌癌组织氮透性调节剂样2(NPRL2)、人异常凝血酶原(PIVKA-Ⅱ)的表达水平及临床意义。方法回顾性分析十堰市太和医院2015年3月至2017年3月收治的68例择期行根治术治疗的结直肠癌患者的临床资料。利用免疫组化检测结直肠癌组织、癌旁组织及正常结肠组织标本中NPRL2、PIVKA-Ⅱ的蛋白表达水平,分析两者与患者临床特征及3年生存率之间的关系,并利用Spearman分析NPRL2、PIVKA-Ⅱ的相关性。结果结直肠癌组织中NPRL2蛋白阳性表达率(32.35%)显著低于癌旁组织(88.24%)及正常结肠组织(90.00%),PIVKA-Ⅱ的蛋白阳性表达率(54.41%)显著高于癌旁组织(2.94%)及正常结肠组织(0),差异有统计学意义(P<0.05);Dukes分期C、D期结直肠癌组织中NPRL2的蛋白阳性表达率(21.88%、8.33%)显著低于A+B期(58.33%),Dukes分期C、D期PIVKA-Ⅱ的蛋白阳性表达率(75.00、100.00%)显著高于A+B期(4.17%),差异有统计学意义(P<0.05);NPRL2的蛋白阳性表达率与肿瘤淋巴转移、肝转移、临床TNM分期有关(P<0.05),PIVKA-Ⅱ的蛋白阳性表达率与肿瘤的肝转移、临床TNM分期有关(P<0.05);68例结直肠癌患者有32例患者死亡,3年生存率为52.94%。NPRL2蛋白阳性表达患者的3年生存率(77.27%)显著高于蛋白阴性表达(41.30%),PIVKA-Ⅱ蛋白阳性表达组患者的3年生存率(37.84%)显著低于蛋白阴性组(70.97%),差异有统计学意义(P<0.05);Spearman相关分析结果显示,结直肠癌组织中NPRL2蛋白与PIVKA-Ⅱ蛋白的阳性表达率呈负相关(r=-0.438,P<0.05)。结论在结直肠癌组织中NPRL2表达降低、PIVKA-Ⅱ表达升高,且与临床病理参数有一定的关联,同时对于预后评估也有一定的参考价值。Objective To study expression levels and clinical significance of nitrogen permease regulator-like 2(NPRL2)and prothrombin induced by vitamin K absence or antagonist-Ⅱ(PIVKA-Ⅱ)in colorectal cancer tissues.Methods The clinical data of 68 patients with colorectal cancer who underwent elective radical surgery and were admitted to Taihe hospital from March 2015 to March 2017 were retrospectively analyzed.Immunohistochemistry was applied to detect expression levels of NPRL2 and PIVKA-Ⅱprotein in colorectal cancer tissues,adjacent tissues and normal colon tissues.The relationship between the two and clinical features,3-year survival rate was analyzed.The correlation between the two was analyzed by Spearman.Results The positive expression rate of NPRL2 protein in colorectal cancer tissues was significantly lower than that in adjacent tissues and normal colon tissues(32.35%vs.88.24%vs.90.00%),while positive expression rate of PIVKA-Ⅱprotein was significantly higher than that in adjacent tissues and normal colon tissues(54.41%vs.2.94%vs.0)(P<0.05).The positive expression rate of NPRL2 protein in colorectal cancer tissues at stage of C and D(21.88%,8.33%)Dukes staging was significantly lower than that at stage A+B(58.33%),while positive expression rate of PIVKA-Ⅱprotein at stage of C and D(75.00,100.00%)was significantly higher than that at stage A+B(4.17%)(P<0.05).The positive expression rate of NPRL2 protein was correlated with tumor lymph metastasis,liver metastasis and clinical TNM staging(P<0.05).The positive expression rate of PIVKA-Ⅱprotein was correlated with tumor liver metastasis and clinical TNM staging(P<0.05).Of the 68 patients with colorectal cancer,there were 32 cases dead.The 3-year survival rate was 52.94%.The 3-year survival rate in patients with positive expression of NPRL2 protein was significantly higher than that with negative expression(77.27%vs.41.30%),while 3-year survival rate in patients with positive expression of PIVKA-Ⅱprotein was significantly lower than that with negative expre
