摘 要:目的探究微小RNA-21(miR-21)和第10号染色体同源缺失性磷酸酶-张力蛋白(PTEN)在肺癌脑转移患者血清中表达水平与临床病理特征及预后的关系。方法以238例肺癌患者为研究对象,根据诊断结果将患者分为肺癌脑转移组78例和肺癌非脑转移组160例,检测两组患者血清中miR-21、PTEN水平,并分析其表达水平与患者临床病理特征及预后的关系。结果肺癌脑转移组患者血清miR-21表达水平显著高于肺癌非脑转移组(P<0.05),PTEN蛋白表达水平显著低于肺癌非脑转移组(P<0.05)。miR-21、PTEN表达水平与肺癌脑转移组患者组织分化程度、脑转移病灶数有关(P<0.05)。miR-21低表达、PTEN高表达肺癌脑转移患者无进展生存率和总生存率分别高于miR-21高表达患者、PTEN低表达患者(P<0.05);COX分析结果显示miR-21、PTEN表达水平、组织分化程度是肺癌脑转移患者不良预后发生的独立危险因素(P<0.05)。结论miR-21在肺癌脑转移患者血清中表达上调,PTEN蛋白表达下调。Objective To investigate the correlations of the expressions of serum micro RNA-21(miR-21)and phosphatase and tension homolog deleted on chromosome ten(PTEN)with the clinicopathological characteristics and prognosis of patients with brain metastasis from lung cancer.Methods Totally 238 patients with lung cancer were treated in our hospital,and they were divided into brain metastasis group(78 cases)and non-brain metastasis group(160 cases).Levels of serum miR-21 and PTEN were measured in two groups,the correlations of their expression levels with clinicopathological characteristics and prognosis were analyzed.Results The expression level of serum miR-21 in lung cancer patients with brain metastasis was significantly higher than that in lung cancer patients without brain metastasis(P<0.05);and the expression level of PTEN was significantly lower than that in lung cancer patients without brain metastasis(P<0.05).The expression levels of miR-21 and PTEN were correlated with the degree of tissue differentiation,tumor stage and the number of brain metastases in lung cancer patients with brain metastasis(P<0.05).The progression-free survival rate and overall survival rate of patients with brain metastasis of lung cancer,and with low expression of miR-21 and high expression of PTEN were significantly higher than those with high expression of miR-21 and low expression of PTEN(P<0.05).COX analysis showed that the expressions of miR-21,PTEN and tissue differentiation were independent risk factors for adverse prognosis of lung cancer patients with brain metastasis(P<0.05).Conclusion The expression of miR-21 is up-regulated and the expression of PTEN is down-regulated in the serum of patients with brain metastasis from lung cancer.
