经皮肤泪囊鼻腔吻合术高频针状电极切口的效果

摘　　要：目的评价经皮肤泪囊鼻腔吻合术高频针状电极切口的效果.方法回顾性分析商丘市第一人民医院2017年1月至2019年9月158例(158只眼)慢性泪囊炎的临床资料.试验组91例(91只眼)行经皮肤泪囊鼻腔吻合术高频针状电极切口者同期行,普通手术刀切口者67例(67只眼)作为对照组.两组均于术后1个月拔出支撑管,拔管后随访3个月.结果试验组术中出血量和手术时间平均值分别为(25.4±6.7)ml和(70.6±11.7)分钟,均少于对照组的(89.3±8.2)ml和(110.4±14.3)分钟(t=53.828,19.219;P=0.004,0.004).拔管后3个月,试验组治愈70例,好转14例,无效7例,有效率92.3%(84/91);对照组治愈44例,好转10例,无效13例,有效率80.6%(54/67),两组间有效率比较差异有统计学意义(χ^2=4.787,P=0.029).结论经皮肤泪囊鼻腔吻合术高频针状电极切口能明显减少术中出血,缩短手术时间,提高手术成功率.Objective To evaluate the clinical efficacy of the insection with high frequency needle electrode in percutaneous dacryocystorhinostomy.Methods The clinical data of 158 eyes of 158 patients with chronic dacryocystitis from Jan.2017 to Sep.2019 in the First People’s Hospital of Shangqiu were analyzed retrospectively.Ninety-one eyes of 91 cases in the trial group were treated with insection with high frequency needle electrode and 67 eyes of 67 patients in the control group were treated with common scalpel during operation.The silicone stent tube was extracted at one month after the operation in both groups.These patients were followed up for 3 months.Results The bleeding volume and operative time in the trial group were(25.4±6.7)ml and(70.6+11.7)minutes respectively,and those in the control group were(89.3±8.2)ml and(110.4±14.3)minutes respectively.There were significant differences between the two groups(t=53.828,19.219;P=0.004,0.004).Three months after extubation,70 cases were cured,14 cases were improved,and 7 cases were ineffective in the trial group.The effective rate was 92.3%(84/91).In addition,44 cases were cured,10 cases were improved,and 13 cases were ineffective in the control group.The effective rate was 80.6%(54/67).The difference between the two groups was statistically significant(χ^2=4.787,P=0.029).Conclusion The application of insection with high frequency needle electrode for the treatment of chronic dacryocystitis during percutaneous dacryocystorhinostomy can significantly reduce the amount of intraoperative bleeding,shorten the operation time,and improve the success rate of the operation.
