归脾合剂联合盐酸托莫西汀治疗儿童注意缺陷多动障碍心脾两虚证疗效观察

摘　　要：目的探讨归脾合剂联合盐酸托莫西汀治疗儿童注意缺陷多动障碍心脾两虚证的疗效及对促肾上腺皮质激素(ACTH)、皮质醇(Cor)、多巴胺(DA)、去甲肾上腺素(NE)水平的影响。方法筛选2017年6月—2018年6月在江南大学附属医院就诊的注意缺陷多动障碍患儿120例作为研究对象,随机分为对照组和观察组各60例。对照组给予盐酸托莫西汀胶囊治疗,观察组在对照组基础上口服归脾合剂(10~20 mL/次,3次/d),2组患儿均连续治疗3个月。比较2组治疗前后心脾两虚证症状评分、Conners父母用症状问卷(PSQ)评分、注意缺陷多动障碍筛查量表(SNAP-IV)评分,评估2组治疗效果,检测2组治疗前后血清ACTH、Cor、DA、NE水平。结果治疗3个月后,2组心脾两虚证症状各项评分、PSQ各维度评分、SNAP-Ⅳ各项评分均显著降低(P均<0.05),且观察组均显著低于对照组(P均<0.05);观察组的临床总有效率为83.3%(50/59),对照组为64.4%(38/60),观察组显著高于对照组(P<0.05);2组血清Cor、DA、NE水平均显著升高(P均<0.05),且观察组均显著高于对照组(P均<0.05)。2组治疗3个月后ACTH水平未见明显变化(P均>0.05)。结论归脾合剂联合盐酸托莫西汀治疗注意缺陷多动障碍心脾两虚证患儿可更有效地改善临床症状,提高治疗有效率,可能与上调血中Cor、DA、NE水平有关。Objective It is to investigate the efficacy of Guipi mixture combined with topoxetine hydrochloride on child attention deficit hyperactivity disorder with syndrome of deficiency of heart and spleen and its effects on the levels of corticotrophin(ACTH),cortisol(Cor),dopamine(DA)and norepinephrine(NE).Methods One hundred and twenty children with attention deficit hyperactivity disorder treated in the Affiliated Hospital of Jiangnan University from June 2017 to June 2018 were screened as the subjects,they were randomly divided into the control group and the observation group,60 cases in each group.The control group was treated with topoxetine hydrochloride capsules,the observation group was treated with Guipi mixture(10~20 mL/time,tid)based on the control group.Both groups were continuously treated for 3 months.The scores of symptoms of syndrome of heart and spleen deficiency,Conners parents symptom questionnaire(PSQ),and attention deficit hyperactivity disorder screening scale(SNAP-Ⅳ)before and after treatment were compared between the two groups,the efficacy was evaluated in the two groups,and the levels of serum ACTH,Cor,DA and NE were detected in both groups.Results After 3 months of treatment,the scores of each symptom of syndrome of heart and spleen deficiency,each item of PSQ and SNAP-Ⅳof the two groups were significantly decreased(P<0.05),and the scores in the observation group were significantly lower than those in the control group(P<0.05).The total effective rate in the observation group was 83.3%(50/59),and the rate in the control group was 64.4%(38/60),the total effective rate in the observation group was obviously higher than that in the control group(P<0.05).After 3 months of treatment,the serum levels of ACTH,Cor,DA and NE in the two groups were significantly increased(P<0.05),and the levels in the observation group were higher than those in the control groups(P<0.05).There was no significant change of ACTH after treatment in the two groups(P>0.05).Conclusion Guipi mixture combined with topoxetin
