非酸反流引起胃食管反流烧心症状的特点

摘　　要：目的分析非酸反流引起烧心症状的特点。方法收集河南科技大学第一附属医院消化内科烧心的胃食管反流病患者的资料,由胃镜结果分为反流性食管炎(RE)、非糜烂性胃食管反流病(NERD)两组,进行食管多通道腔内阻抗-pH(MII-pH)监测。结果 RE组非酸反流次数、弱酸反流次数、总反流次数、非酸反流所占百分比均低于NERD组;NERD组非酸反流次数、弱酸反流次数、总反流次数、非酸反流所占百分均比高于RE组,差异有统计学意义(P<0.05);混合反流、近端反流在与症状相关的非酸反流事件中所占比值高于在与症状无关的非酸反流事件中的比值,差异有统计学意义(P<0.05)。结论非酸反流可能是导致烧心的重要因素之一,混合性反流和食管近端反流是影响非酸反流被感知的主要因素。Objective To analyze the characteristics of heartburn symptoms caused by non-acid reflux. Methods The patients with gastroesophageal reflux disease, who were heartburned in the department of gastroenterology of the First Affiliated of Hospital of Henan University of Science and Technology, were divided into reflux esophagitis(RE) group and non-erosive gastro-esophageal reflux group(NERD), and monitored with the esophagus multichannel intraluminal impedance-pH(MII-pH). Results The number of non-acid reflux,weak acid reflux,total reflux, the percentage of acid reflux in the NERD group were higher than those in the RE group,and the differences were statistically significant(P<0.05, respectively). The proportion of mixed reflux and proximal reflux in the non-acid reflux events related to symptoms,was higher than that in non-acid reflux events unrelated to symptoms(P<0.05). Conclusion Non-acid reflux may be one of the important factors leading to heartburn. Mixed reflux and proximal esophageal reflux are the main factors affecting the perception of non-acid reflux.
