摘 要:目的通过分析急性白血病患者发病时以及缓解时外周血CD3^-CD56^+NK细胞及CD3^+CD56^+NKT样淋巴细胞的数量以及功能变化,进一步了解急性白血病患者免疫功能状态。方法使用流式细胞分析外周血CD3^-CD56^+NK细胞及CD3^+CD56^+NKT样淋巴细胞的数量,同时分析上述两群细胞内的干扰素表达水平来评估细胞的功能。结果CD3^+CD56^+NKT样淋巴细胞数量在急性白血病发病时显著增加,经化疗获得完全缓解后数量又下降到正常健康人水平,并且无论在发病时还是缓解时CD3^+CD56^+NKT样淋巴细胞内干扰素水平均明显下降。CD3^-CD56^+NK细胞的变化有所不同,在发病时数量以及功能改变均不明显,而在疾病缓解时,急性淋巴细胞白血病患者CD3^-CD56^+NK细胞的数量下降明显,急性髓系白血病患者CD3^-CD56^+NK细胞的功能下降明显。结论急性白血病患者发病时可能产生免疫反应,但免疫细胞功能缺陷可能参与疾病的发生进展,其中CD3^+CD56^+NKT样淋巴细胞的变化可能与疾病的关系更密切,且急性髓系白血病患者免疫细胞功能异常更明显。Objective By analyzing the number and functional changes of CD3^-CD56^+NK cells and CD3^+CD56^+NKT-like lymphocytes in peripheral blood during onset and remission in patients with acute leukemia,the immune function status of patients with acute leukemia was further understood.Methods Flow cytometry was used to analyze the number of CD3^-CD56^+NK cells and CD3^+CD56^+NKT-like lymphocytes in peripheral blood,and interferon expression levels in the above two groups of cells were also analyzed to evaluate the cell function.Results Studies have shown that the number of CD3^+CD56^+NKT-like lymphocytes increased significantly at the onset of acute leukemia,and decreased to the level of normal healthy people after complete remission through chemotherapy.Moreover,it was found that the interferon level of CD3^+CD56^+NKT-like lymphocytes decreased significantly both at the onset and remission(contradictory).The changes of CD3^-CD56^+NK cells were different,and the number and function of CD3^-CD56^+NK cells were not obvious at the onset of the disease,while the number of CD3^-CD56^+NK cells decreased significantly in patients with acute lymphoblastic leukemia,and the function of CD3^-CD56^+NK cells decreased significantly in patients with acute myeloid leukemia.Conclusion In patients with acute myeloid leukemia,there may be an immune response,but the immune cell function deficiency may be involved in the progression of the disease,among which the changes of CD3^+CD56^+NKT-like lymphocytes may be more closely related to the disease,and the abnormal immune cell function is more obvious in patients with acute myeloid leukemia.
