探讨醋酸泼尼松片联合抗痨药物治疗结核性胸膜炎的临床效果

摘　　要：目的探讨结核性胸膜炎应用醋酸泼尼松片与抗痨药物联合治疗的临床效果。方法选取连云港市第四人民医院2015年9月至2019年7月收治的结核性胸膜炎患者40例为研究对象,按照入组奇偶顺序分为观察组与对照组,各20例。对照组患者采用常规抗痨药物治疗,观察组患者在抗痨治疗基础上加用醋酸泼尼松片治疗。比较两组患者的临床效果、全身中毒缓解时间、胸腔积液吸收时间、住院时间以及不良反应发生情况。治疗后半年随访复查CT观察胸膜增厚与胸膜黏连情况。结果观察组患者治疗总有效率为95.00%高于对照组的70.00%;观察组患者全身中毒缓解时间、胸腔积液吸收时间、住院时间均短于对照组;治疗后半年随访复查CT检查,发现观察组患者胸膜增厚与胸膜黏连率均低于对照组,差异有统计学意义(P<0.05)。两组患者均有不良反应发生,比较差异无统计学意义(P>0.05)。结论结核性胸膜炎应用醋酸泼尼松片联合抗痨药物治疗,相比单用抗痨药物治疗,不仅可提高临床效果,而且缩短全身中毒缓解时间、胸腔积液吸收时间、住院时间,更好地避免胸膜增厚与胸膜黏连,且安全性较高。Objective To investigate the clinical effect of prednisone acetate tablets combined with anti tuberculosis drugs in the treatment of tuberculous pleurisy.Methods Totally 40 patients with tuberculous pleurisy admitted to the Fourth People's Hospital of Lianyungang City from September 2015 to July 2019 were enrolled in the study,The patients were divided into observation group and control group according to parity order,20 cases in each group.The control group was treated with conventional anti tuberculosis drugs,while the observation group was treated with prednisone acetate tablets on the basis of anti tuberculosis treatment.The clinical effect,remission time of systemic poisoning,pleural effusion absorption time,hospitalization time and adverse reactions were compared between the two groups.Half a year after treatment,CT was followed up to observe the pleural thickening and pleural adhesion.Results The total effective rate of the observation group was 95.00%,which was higher than 70.00%of the control group;The remission time,pleural effusion absorption time and hospitalization time of the observation group were shorter than those of the control group;Half a year after treatment,the follow-up CT examination showed that the pleural thickening and adhesion rate of the observation group were lower than those of the control group,The above differences were statistically significant(P<0.05).Adverse reactions occurred in both groups,but the difference was not statistically significant(P>0.05).Conclusion The application of prednisone acetate tablets combined with antituberculosis drugs in the treatment of tuberculous pleurisy can not only improve the clinical effect,but also shorten the remission time of systemic poisoning,the absorption time of pleural effusion and hospitalization time,and better avoid pleural thickening and pleural adhesion,and has good safety.
