牙科手术显微镜在老年磨牙根管数探查及治疗中的意义

摘　　要：目的观察牙科显微镜显微根管治疗(DOM)对老年根管治疗期间磨牙根管的检出率及治疗效果的影响。方法选取2017年10月至2018年10月太原市第二人民医院口腔科就诊的老年磨牙病变患者144例,患牙144颗,随机数字表法分为常规组(72例)和显微根管治疗(DOM)组(72例),比较两组患者磨牙根管的检出率及1年随访的治疗效果。结果常规组4根管检出8例(11.11%),DOM组4根管检出22例(30.56%),组间比较差异有统计学意义(P<0.05);常规组5根管检出0例,DOM组5根管检出2例(2.67%),组间比较差异有统计学意义(P<0.05)。对两组磨牙>3根管的总检出率进行χ^2检验,显示差异有统计学意义(P<0.05)。两组患者均接受1年随访。常规组5例治疗失败患者中有3例是因根管遗漏而造成的;DOM组2例治疗失败患者中有1例是因根管遗漏而造成的。结论牙科显微镜可显著提高老年磨牙根管治疗的效果。Objective To investigate the effect of dental microscope on the detection rate and treatment effect of molar root canal during root canal treatment in the elderly.Methods From October 2017 to October 2018,144 elderly patients with molar lesions were selected from the Dental Department of Taiyuan Second People's Hospital,and 144 patients with molar lesions were randomLy divided into the routine group(75 cases)and DOM group(69 cases),and the detection rate of molar root canal and the treatment effect of one-year follow-up were compared between the two groups.Results Eight cases(11.1%)of 4 root canals were detected in the conventional group,and 22 cases(30.6%)of 4 root canals were detected in the DOM group,with significant difference(P<0.05).There were 0 cases of 5 root canals detected in the conventional group and 2 cases of 5 root canals detected in the DOM group(2.7%),with significant difference(P<0.05).χ^2 test was performed on the total detection rate of molars>3 root canals in two groups,and the difference was significant(P<0.05).Patients in both groups were followed up for 1 year.In the conventional group,3 of the 5 patients with treatment failure were caused by root canal omission.In the DOM group,1 of the 2 patients with treatment failure was caused by root canal omission.Conclusion Dental microscope can significantly improve the effect of root canal treatment in the aged molars.
